Finding 2023-001 Cash Management - Heightened cash monitoring payment method Federal Agency Name: Department of Education Program Name: Student Financial Aid Cluster ALN #84.063 - Federal Pell Grant Program ALN #84.007 - Federal Supplemental Educational Opportunity Grants ALN #84.033 - Federal Work-Study Program ALN #84.268 - Federal Direct Student Loans Finding Summary: During testing of cash management, which includes disbursing of Title IV program funds under HCM1, a sample of 11 students was selected from the population of students receiving Title IV funding during fiscal year 2023. From this selection of students, the following deficiencies were noted where the College received Title IV payments from the Department of Education before either applying the funds to the students account or clearing any credit balances owed to the student/parent that were created by applying the funds to the students account. • Pell Grants – 10 of the 19 disbursements • Subsidized Loans – 17 of the 30 disbursements • Unsubsidized Loans – 18 of the 29 disbursements • Plus Loans – 4 of the 6 disbursements • FSEOG Grants – 9 of the 14 disbursements Responsible Individuals: Bryan Tarrant (Director of Operations) and Ryan Apple (Financial Aid Director) Corrective Action Plan: Management acknowledges the importance of continued training for staff to strengthen their knowledge of cash management practices and that processes and procedures relating to cash management are continually reviewed and updated. Anticipated Completion Date: We anticipate management’s review of practices and processes and additional training to be completed by December 31, 2023. The College anticipates continued review of policies and procedures on a yearly basis and additional training as the need arises.

Finding 2023-001 - Public Housing Tenant Account Receivables - Eligibility - Internal Control Over Tenant Terminations and Nonpayment of Rent Low Income Public Housing Program ALN #14.850 - Noncompliance and Material Weakness Corrective Action Plan: The following account collection management practices will be implemented immediately: 1. Property Managers will review all delinquent accounts on the 8th of each month, at which time a Late Rent Meeting will be conducted with perspective tenants to discuss ca use, and or a payment arrangement. 2. On the 14th of each month, all delinquent accounts will receive a Final Notice regarding nonpayment of rent. (With the exception of an approved payment arrangement.) 3. Court papers will be filed in County Court on the 18th of each month for all delinquent accounts, with the exception of those with approved payment arrangements. 4. All tenants that were not served for County Court will be filed in Justice Court, for non-payment of rent and or removal of occupied units. Person Responsible: Ronald J. Upon learning of the possibility of frauduelent activity, the University began an internal audit review and all activity on the grant was stopped. Throughout the process, the University coordinated with the Ohio Department of Development. The internal audit procedures led to the determination that $209,101 was incorrectly reported by the program advisor and was not detected by the program director. These funds were returned to the Ohio Department of Development on October 11, 2023. The program has been termianted and program income returned. The individuals involved with this program are no longer employees of the University. The University is in the process of seeking reimbursement from the former employee. An internal controls questionnaire was prepared and reviewed for the other Small Business Development Center (SBDC) program noting no areas of concern. The FY24 internal audit plan will include additional review of the remaining SBDC program as well as review of controls within the department which previously managed the program noted in the finding. In addition, training related to roles and responsibilities for supervisors/approvers will be provided in FY24 to emphasize the guidance provided in the grants manual. Finding 2023-001 – Reporting The BOCES concurs with the finding 2023-001. Corrective Action: To correct this in the future 2 steps will be implemented by the BOCES: 1. A manual total of meals from the Etrition claim reports will be calculated and compared to the total meals on the CNMS claim; and 2. The School Food Service Director and Director of Shared Food Services will review and verify each other's work. Additional checks and balances will be put in place immediately so that reliance is not solely on the computerized system. Finding and Recommendation - Finding: 2023‐001 (repeat finding 2022-002, 2021‐001 & 2020‐002) Finding Type: Noncompliance with laws and regulations. Condition: The Academy NSFSA’s fund balance exceeded the allowable three months’ average expenditures balance as of June 30, 2023. The Academy had approximately 4.96 months of expenditures as fund balance as of June 30, 2023. Recommendation: The Academy should ensure it has proper internal controls in place to comply with its annual external reporting requirements in accordance with state law. Corrective Action Plan - The management company for the Academy will work with the Academy leadership to increase expenditures in a manner necessary to spend down the excess fund balance in an allowable and timely fashion. The spend down plan will include improvements to the food service program, including adding an additional food service support position. The number of salad bar offerings and daily hot breakfast options will be increased for all grade levels. The Academy will also explore allowable options for spending funds on supplies, equipment and initiatives that will create sustainable improvements to the food service program for future years. I am acknowledging the finding of the Federal Audit team in which an error in my spreadsheet was documented resulting in requesting a recurring expenditure on two different pay requests. The correction was made the day of the audit through coding other expenditures matching the qualifying expenditures. In the future, the district spreadsheets will include review by the bookkeeper and superintendent to ensure the fund pay requests are correct and not repeated. By multiple review and the addition of PO number and date of pay request this will easily define a possible "doubling up" of items for a pay request. This was one finding and all other accounts reviewed were correct and accurate. Additional expenditures were corrected and easily matched the grant funds obtained through reimbursement. The findings are numbered consistently with the numbers assigned in the Schedule. FINANCIAL STATEMENT FINDINGS None FEDERAL AWARD FINDINGS AND QUESTIONED COSTS 2023-001 Coronavirus State and Local Fiscal Recovery Funds – Assistance Listing No. 21.027 Significant Deficiency in Internal Control over Compliance and Noncompliance – H. Period of Performance Recommendation: The Auditor recommended the IFA implement procedures to ensure the disbursement review process is operating effectively. Planned Corrective Actions: The Indiana Finance Authority (IFA) has procedures in place to assure the appropriate use of the federal funds the IFA manages. IFA oversight includes the robust review process of all disbursements which includes IFA engineers and finance staff. With respect to the matter your letter references, the IFA reviewed our existing Standard Operating Procedures and have edited them to reflect the date of the period of performance as part of the checklist for the program. The new IFA Program Manager has been updated on the procedures and the importance of being in compliant with the federal guidelines. The funds were corrected subsequent to year end and paid with operating revenues during fiscal year 2024. Name of Responsible Individual: Andy Mueller, Director of Financial Reporting and Planning Corrective Action: Through 9/13/22, PCOM used a Banner generated report to draw down direct loan funds. PCOM found there was an error with the Banner report causing excess funds to be drawn on 6/30/22, 8/2/22 and 8/31/22. With the assistance of the Student Services department a new process to draw direct loans began in November 2022. The process compares disbursements recorded by the Department of Education's Common Origination and Disbursement (COD) website to that of the disbursements paid in PCOM’s ERP information system. The finance department ensures the amount to be drawn from COD is either less than or matches the available funds on the COD site. Since the implementation of the new process there have been no drawdowns that resulted in an excess cash situation. Anticipated Completion Date: November 30, 2022

This will allow the agency to have one finance person creating the reimbursement calculations and a second finance person reconciling the calculations. The Agency is updating its process to calculate the indirect costs in accordance with the revised notice of award (NOA) dated March 10, 2023. Additionally, the Agency will provide further training to all individuals involved in the financial management of federal awards. On a monthly basis, the calculation of indirect costs eligible for reimbursement under this award will be compared to the indirect costs allowed for in the NOA. This calculation will be secondarily reviewed by an individual having financial oversight on federal awards to ensure that any reimbursement request is computed in accordance with the NOA. The reimbursement request will then be submitted only after this verification has been completed. The Organization will continue to provide ongoing training to clinic staff who evaluate the sliding fee application at its clinic locations. The training consists of reviewing sliding fee program policies and procedures along with all applicable patient forms, sliding fee scale, and patient eligibility. The following quality control measures to ensure compliance have been implemented effective April 30, 2023; 1. Front Desk Peer Review of sliding fee application and verification of patient income and family size. 2. Enhance training materials to support Front Desk Staff with assessing sliding fee applications. 3. Quarterly feedback to Front Desk Staff based on sliding fee applications reviewed.
