The Organization acknowledges the lack of an internal control process to assure all individuals claimed on federal grant activities met Temporary Assistance for Needy Families (TANF) eligibility requirements. To address this deficiency, we have developed internal controls to prevent ineligible individuals from being claimed: ? A second person, the Director of Quality & Process Improvement, reviews the data entered by the employee responsible for preparing program reports to verify TANF eligibility for each individual claimed on the grant ? Any errors discovered are corrected before the program report is approved ? After the employee?s work has been reviewed and any errors have been corrected, the Director of Quality & Process Improvement approves and submits the report The Organization will continue to implement an internal control system requiring that a second person reviews eligibility requirements before claiming individuals on the federal TANF grant.

Timely Submission of Required Reporting Packages Federal Assistance Listing Number Name of Federal Program 14.129 Mortgage Insurance Nursing Homes, Intermediate Care Facilities, Board and Care Homes and Assisted Living Facilities Criteria The Home is required to file its Owner Certified Real Estate Assessment Center (REAC) financial statements by March 31, 2023, in accordance with U.S. Department of Housing and Urban Development (HUD) regulations. Condition The Home?s Certified REAC financial statement submission due March 31, 2023, was not filed timely. Questioned Costs None Context While performing the audit, we noted that the March 31, 2023 Owner?s Certification submission to REAC was not filed timely. Effect The failure to submit the Owner?s Certification on a timely basis caused the Home to be noncompliant with the reporting requirements of HUD. Recommendation We recommend that the Home ensure the filing of its HUD report is performed in accordance with HUD reporting requirements. Management's Response Management acknowledges that owner certified financial data is required to be submitted through HUD?s REAL ESTATE ASSESSMENT online system by March 31 of each year for the preceding calendar year. To ensure that this deadline is adhered to each year going forward the CFO or designee will create an aggressive closing schedule so that accurate financial information is available by February 15th of each year from which to create the owner certified submission.

Recommendation: Management should deposit the amount or request HUD's approval for a waiver. Action Taken or Planned: Due to pending cash requirements, we requested HUD's approval to waive the deposit requirement, however an answer was not obtained. We will again seek w waiver for the current year and, if not approved, we will follow the HUD Account Executive?s instructions on how to resolve the matter. 2022-02: Significant Deficiency ? Program income compliance and controls Federal Agency: Department of Health and Human Services Program: Certified Community Behavioral Health Clinic Demonstration Grant Assistance Listing Number: 93.829 Management agrees that during the year ended December 31, 2022, Flushing did not have a formal control in place to identify, monitor and report program income collected from providing mental health counseling services to patients under the grant. Management has contacted The Department of Health and Human Services to inform them of this finding. Medisys Health Network, which includes Jamaica Hospital and Flushing, is the recipient of various federal grants, including another grant with program income requirements which was identified as a result of management?s review of the awards and for which controls have been designed and implemented to ensure compliance with the requirement. We believe our oversight of this compliance requirement was an isolated situation because the NoA only included one brief sentence regarding program income. Flushing will implement the following process to formalize controls related to the program income compliance requirement for the grant. 1) Management will review monthly charge/income reports for each clinician hired under the grant to keep track of the program income related to the grant. Management has started reviewing the program revenue and will set up quarterly reviews with the program director. 2) Management will keep track of all program income related to the grant and compare the income to the current expenses, and retain documentation supporting how the program income was used to further eligible project objectives prior to requesting reimbursement from the agency under the grant. 3) These controls and procedures will be implemented by the end of the 3rd quarter of 2023. Allowable Costs Recommendation: We recommend the County review time and effort records to ensure overtime is not charged to Federal grants on days in which vacation and sick time is used. More detailed reporting of the days in which the vacation and sick days are used and the overtime days would assist with this process. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: The public health department is in the process of training new employees that are responsible for payroll and grant allocations. The Finance Manager has begun conversations with the new employee and the public health administrator on documentation and review. It was discussed that no benefit time such as vacation or sick be charged to a grant. If there are allowances within a grant for benefit time to be charged, there must be proper documentation and detailed approval by the public health board. This will be implemented immediately. Views of Responsible Officials: RFE/RL Management fully agrees on the need to ensure our policies and procedures demonstrate adherence to the requirements of the Patriot Act. The process for new employee screening will be reviewed and modified to ensure documented compliance. New vendor and contractor screening is conducted and now fully documented during the initial procurement/contracting process in our Prague office. Additionally, RFE/RL has implemented a process for regular, automated Office of Foreign Asset Control (OFAC) screening of all vendors in FY23. A similar process for regular, automated System Award Management (SAM) screening is in process. Once implemented, these systems will ensure that all vendors on RFE/RL?s supplier list will be reviewed annually.

By the end of FY 2023, Management intends to have identified the root causes of these anomalies, including bringing in outside experts to examine systems, workflows, personnel capabilities, policies and practices, and training protocols. Based on that analysis, Management expects to implement stronger controls and practices in FY 2024.

Views of Responsible Officials: RFE/RL?s Finance management team understands the importance of accurate and timely account reconciliations. Asset and liability account reconciliations are prioritized, prepared, and reviewed on a schedule in line with the level of activity in the account and in accordance with the best use of limited staff resources. Accounts with a large quantity of monthly transactions or significant dollar amounts are reconciled monthly; those with little activity may be reviewed quarterly or annually. An accounting close and reconciliation management tool that will create more accountability and insight into account analysis across locations is being implemented in FY23. Staff will be trained to ensure their reconciliation provides clear information to any outside finance professional as to the items that make up the balance in the account and amounts are to be easily traceable to support documentation that they can provide upon request. Auditors will have access via the software to all account reconciliations upon demand.

Management will review standards and requirements annually to ensure that all of our contracts are in compliance with federal guidelines. Management will also develop a guide/checklist to follow to ensure that all criteria and requirements are met for future federal grants. Anticipated Completion Date: April 1, 2023

U.S. Department of Education 2022-003: Student Financial Aid Cluster ? Student Eligibility and Awarding ? Assistance Listing Number: Various Recommendation: We recommend that the College implements a process that will ensure all Title IV funds are awarded at proper amounts. Action taken in response to finding: This student was awarded an incorrect amount because a subsequent ISIR transaction was received but the Pell was not recalculated on the basis of the new information. After this discovery, we have taken the following actions in response: ? We examined our ISIR import process to make sure that our means of communicating locked transactions was functioning correctly. We found that our system for monitoring new transactions was deficient; if a set of conditions were aligned, a new transaction could slip by our notice. Implemented by August 2022. ? We added another layer of review wherein the output of both the messages we receive from our third-party verification partner and our internal reports associated with importing ISIRS are examined on a regular basis. New transactions on students with a current locked transaction are reported to staff members for further review. Implemented by August 2022. ? We wrote an ad hoc report that allows us to identify subsequent ISIR transactions and will run it regularly to reduce the likelihood of this issue occurring again. Implemented by August 2022.
