Adults? & Children?s Alliance is submitting the following Corrective Action Plan for Significant Deficiency found during audit for FY22 (10/1/2021-09/30/2022) Found in Section II ? Financial Statements Findings 2022-001 ? Lack of Segregation of Accounting Duties (Repeat finding 2021-001) Criteria: Proper internal control structure includes review of journal entries, bank reconciliations and the schedule of expenditures of federal awards, as well as an adequate system for recording and processing entries to the financial statements, in accordance with generally accepted accounting principles. Condition: The limited number of staff in the accounting department results in certain functions that are not properly segregated which normally would enhance internal control, including the lack of review of journal entries, bank reconciliations, and the schedule of expenditures of federal awards. Cause: The internal control structure does not provide an appropriate segregation of duties for the financial reporting process. Effect: Although this condition is not unusual for an entity the size of the Organization, the condition may affect the Organization's ability to initiate, record, process, and report financial data consistent with the assertions of management in the financial statements. Recommendation: It is the responsibility of management and those charged with governance to determine whether to accept the risk associated with this condition because of cost or other conditions. We recommend the Organization evaluate current procedures and segregate where possible and implement compensating controls.Responsible Official?s Response: Management will evaluate current procedures and segregate where possible and implement compensating/alternative controls appropriately according to staffing and budget. Corrective Actions: ACA will continue to work with Bottom Line Accounting Services when finances do not align Lisa Dunlap, the Executive Director, works with Bottom Line Accounting Services to find resolution. Contact person: Lisa Dunlap Lisa.dunlap@acainc.org 651-481-9320 2022-002 - Reporting Information on the SEFA Criteria: 2 CFR Part 200.510(b) states that the auditee must prepare a schedule of expenditures of federal awards for the period covered by the auditee's financial statements which must include the total federal awardsexpended. Federal program and award identification must include, as applicable, the Assistance Listing Number and title, the federal award identification number and year, the name of the federal agency, and the name of the pass-through entity, if any. This information enables the auditee to reconcile amounts presented in the financial statements to related amounts in the schedule of expenditures of federal awards. Condition: Management did not have a process in place to prepare a complete schedule of expenditures of federal awards, including identifying COVID-19 funding. The audit firm cannot serve as a compensating control. Cause: Proper processes were not in place for management to prepare the schedule of expenditures of federal awards. Potential Effect: As a result of this condition, there is a higher risk that the schedule of expenditures of federal awards could be incomplete or contain errors that are not detected. Recommendation: The Organization should review its policies and procedures to ensure all expenditures charged to federal grants are properly identified, recorded in the general ledger, and reflected on the schedule of expenditures of federal awards. Responsible Official's Response: Management is now aware that Emergency/Covid funds should have been separated by line when reporting even though from the same source, grant and pass-through grant number. Corrective Actions: SEFA The Schedule of Federal Awards report is completed by Lisa Dunlap with review from Bottom Line Accounting Services. Funding strands will be broken out and identified accordingly by funding type, grant number, pass through grant number as well as identified in general ledger with same information. Meal Counts Federal Program: Assistance listing number 10.558, Child and Adult Care Food Program ? United States Department of Agriculture Compliance Requirement: Eligibility Criteria: A properly designed system of internal control over compliance with the requirements of federal programs allows entities to meet those requirements set forth by the federal government. Under the Child and Adult Care Food Program, the Organization is required to monitor eligibility of meals being reimbursed to providers. Condition: 1 of the 40 providers tested for meal counts had discrepancies. The provider's reimbursement improperly included 2 additional breakfast meal counts. Cause: The Organization noted a deduction of a breakfast count should have been made, however rather than deducting another breakfast count was added resulting in 2 additional breakfast meal counts. Questioned Costs: The results of this noncompliance did not result in any questions costs. Potential Effect: As a result of this condition, there is a higher risk that the provider meal counts are inaccurately reimbursed. Recommendation: The Organization should review its policies and procedures to ensure all provider meals charged to federal grants are properly reflected in the reimbursement request. Responsible Official's Response: This was a human error; management will continue to follow policy and procedures in place to ensure all meals charged to the federal grant are properly reflected in the reimbursement request. Finding 2022-001 Delay in Financial Reporting Audit Finding: Management is responsible for providing timely and accurate financial information. The Center is required to submit the Data Collection Form and the reporting package to the Federal Audit Clearinghouse and the State Controller?s Office, which include the Basic Financial Statements of the Center within the earlier of 30 days after receipt of the auditor?s report, or nine months after the end of the audit period. The Center has experienced delays in the preparation and issuance of the year ended June 30, 2022 basic financial statements and its Single Audit required under Union Guidance. Corrective Action Plan: Due to AVHC's remote location, small size and FQHC status, we have found it extremely challenging to hire accounting staff with the required skills and knowledge to manage our unique organization, so we have successfully outsourced our accounting department for many years. However, when our former outsourced company sold to a large corporation, we began to experience a decline in services. Deadlines were not being met, yet costs were increasing 50% to 100%. In December 2022, a local FQHC began providing accounting services for us under a shared service agreement. Unfortunately, the FY22 audit was not complete at the time of the transition, and though we were under contract with the former consultant to complete the audit work, they were ultimately unable to complete the audit. Staff under the new agreement did not have access to critical historical data required to complete the last few outstanding items, increasing the amount of time to address them. Since FY22 audit work was not part of the new agreement, adequate staffing was not in place to manage the additional work. Management understands how important it is to meet the annual audit deadline. The plan for attaining and maintaining compliance consists of the following actions, many of which are in place: ? Review monthly processes to ensure workpapers are audit ready and that minimal adjustments are required after June financials have been issued. ? Manage staffing levels to ensure experienced staff are available to work with auditors during the annual audit period. ? Identify staff responsible for assisting with audit preparation and conduct regular training to ensure they can efficiently prepare requested documents and address auditor requests. ? Adhere to a pre-planned schedule with built-in time for unexpected delays. ? Begin planning for each audit six months prior to the end of the fiscal year: o Reach out to the selected auditor in January for an Engagement Letter, a PBC list, and to schedule fieldwork. o o Actively work with vendors to ensure all FY invoices are entered no later than the end of July so that a Trial Balance and other initially requested documents are provided to auditors no later than August 15. o o Staff assigned to assist with audit preparation are directed to prioritize audit work from July 1 until completion of audited financials. They will prioritize all requests from auditors, including document and sample requests and responding to questions. o o Any deviation from interim deadlines is to be communicated between accounting staff and auditors for resource planning on both sides. o o Weekly meetings will be scheduled between Management, accounting staff and audit staff at any point that the audit seems to be falling behind the planned schedule, to work through any issues as efficiently as possible. We are confident that full implementation of, and continuing attention to, these measures will ensure we complete future audits on time, beginning with FY23. There were no recommendations or indications to perform a Single Audit, for these purposes we have always been complying. We have previously spoken with the NWA's Organizational Assessment Division office and they are aware of what we have stated before. The organization will establish processes administrative controls to monitor the closing procedures and to allow the process of requesting a single audit, if necessary.

Responsible Individuals: Lori Herrick, CPA, CFE - Associate Vice President of Finance Dr. Eric Gumm - Registrar and Director of the First-Year Program and Academic Development Center Finding 2022-001 Federal Agency Name: U.S. Department of Education Program Name: Student Financial Assistance Cluster CFDA #84.063, 84.268 Finding Summary: In accordance with 34 CFR sections 690.93(b)(2), 682.610, and 685.309(i), Federal Regulations state that institutions are required to report enrollment information. Out of a sample size of 25 students, there were 19 students identified as not having an incorrect Program Enrollment Effective Date and 3 students in which the update for enrollment status was not timely. Corrective Action Plan (CAP): ACU has identified the source and cause of the variance in the program enrollment effective date. The variance is due to the time stamp associated with certain actions within the Banner reporting system. Immediate implementation of system process to change the time status upon the appropriate enrollment change has corrected this error and the timely reporting of status changes. Anticipated Completion Date: The updated procedure was implemented by the beginning of Fall 2022. Responsible Parties: Dr. Eric Gumm is the responsible party as the University Registrar. He will oversee the execution of the corrective action plan. J Rodriquez is the Assistant Registrar and the responsible part for the implementation and execution of the corrective action plan.

HUD has agreed to suspend the monthly required debt service savings deposit effective September 1, 2019. Management is negotiating with HUD to get the past debt service saving deposit requirement suspended permanently.
