Name of Responsible Individual: Aaron Carlson, Executive Director of Financial Aid Corrective Action: Corrective action was taken. While verification was completed properly for each selected student, when changes were not required to the ISIRs of the students, ISIRs were not consistently released to COD. Now that the University is fully operating with its new financial aid system, JFA, we are running a daily process sending up ISIR corrections to COD for all students. Thorough review and training sessions on the verification process have been held with the financial aid team and we will continue to diligently monitor the verification process, including obtaining necessary documentation from students selected for verification and processing ISIR corrections. All residual receipts are required to be deposited in a separate federally insured account within 60 days of the fiscal year-end. Finding: 2022-005 Federal Agency name: Department of Agriculture Program Name: Communities Facilities Loans and Grants Cluster Federal Financial Assistance Listing: #10.766 Compliance Requirement: Special Tests and Provisions Finding summary: Management maintained the reserve amount in the pooled investment fund account which was not established as a separate bookkeeping account nor as a separate bank account. Although the pooled investment funds includes marketable securities backed by the full faith and credit of the United States, based on the portfolio mix of the investment pool, additional cash balances on hand need to supplement the investment pool to adequately fund the reserve. The Organization has excess cash available. Further, there is no secondary level of review being performed over the monthly reconciliation of the reserve account. Management concurs that the subscription period was from June 28, 2022 through June 27, 2023 and the benefits of the subscription service purchased under the award were received outside the grant?s period of performance. However, benefits were in support of Year 3 of the Coronavirus Recovery Project, which was granted by MBDA in January 2022 to start July 1, 2022. In order to have coverage from the start of the project, the subscription was purchased to ensure no break in service during to MBE during Year 3. In the future, as a part of our grant financial process, we will seek written approval from our program manager. In Finding 2022-004, it was reported that the Uniform Data System report submitted to DHHS for the year ended December 31, 2021 contained incorrect data for contained incorrect data for expenses and charges. The expenses were overstated on Table 8A of the UDS report by approximately $682,000. The charges were understated on Table 9D of the UDS report by approximately $680,000. Management recognizes the importance of complying with federal reporting guidelines. In response to Finding 2022-004, efforts will be made to ensure that that expenses and charges are reconciled from the financial reporting system to expenses on the UDS report. This will be implemented by the Chief Financial Officer by January 31, 2023.

CORRECTIVE ACTON PLAN December 31, 2022 Finding No. 2022-001; Name of Responsible Official James Haislip, VP System Office Finance; Management?s Response to Findings - Management agrees with the finding. The issue results from BayCare tracking contract labor time in its timekeeping system and then submitting the hours worked to the contract labor firms to pay the individual. BayCare?s timekeeping system also included hourly rates for each contracted position. Due to fluctuating market conditions, pay rates for contract labor were changing frequently but not updated timely in our timekeeping system. Allowable costs submitted for Provider Relief Funds were based on information from our timekeeping system. Description of Corrective Action - Allowable cost submitted for Provider Relief Funds were based on information from our timekeeping system. The finding was first identified in Reporting Period 2 (RP2) and communicated to management after RP3 was prepared. RP3 included PRF expenses through Q2 of 2022. RP4 included PRF expenses through Q4 2022. Management implemented the prior year Corrective Action Plan (CAP) and as a result the error rate on contract labor incurred in Q3 of 2022 decreased compared to prior year with minimal errors identified. There were no errors identified for Q4 2022. Anticipated Completion Date - CAP was completed in RP5.

Management will also reinforce the importance of employees providing the appropriate invoices or supporting documentation for all expenditures submitted for payment. Expenditures will not be paid without the appropriate supporting documentation. All approvals will be tracked through an online system. Unfortunately, the prevailing wage requirement was not checked as a requirement for this particular job. Requirements normally specific to public school districts carries forward to the specifications issued by our architects, which did not happen this time. Planned Corrective Action: ?BVFB will continue to complete at least two (2) full Physical Inventory Counts each year, at the beginning of the fiscal year in July, and at the end of the calendar year in December. The overall Physical Inventory Process is currently overseen by Ebony Knight, Operations Director, with documented counts completed and documented in pairs of warehouse staff, supervised by BVFB?s Distribution Manager, Tyler Foley. The Operations Director reviews manual counts against current inventory listings, with discrepancies requiring recounts by warehouse staff. Variances are corrected in BVFB?s inventory system by the Operations Director, with assistance from the Finance Team. Variance reports are provided to the Executive Director after each full inventory. oDue Date: Current practice that will continue. ? As BVFB?s inventory becomes more varied and complex, BVFB sees the need for a paid position dedicated to Food Purchase & Inventory at BVFB. A job description is being developed, funds are in the current annual budget and recruitment is to begin. oDue Date: Hire made for this position by June 30, 2023. ?Manual product category inventory checks, with special emphasis on TEFAP products, will be performed monthly. The Operations Director, Ebony Knight, will oversee this process until a Logistics Manager is hired. At the completion of each month, warehouse staff will manually count designated product categories. The Operations Director/Logistics Manager will cross reference manual counts with the Inventory Transaction Report (ITR) run from BVFB?s inventory management system. BVFB?s Operations Director then reconciles any discrepancies in BVFB?s inventory system. oDue Date: Implemented January 2023. ?BVFB?s Distribution Manager, Tyler Foley, provided training to all warehouse staff on specific procedures for 1) accurately reporting damaged/lost product, including required documentation when a discrepancy is identified. Damaged/lost product should be reported to the Logistics Manager (currently being filled by the Operations Director) during the receiving process and to the Distribution Manager during the distribution process. oDue Date: First training occurred 1/17/2023. Subsequent training for new employees or new hires done by the new Logistics Manager will happen as need. ?BVFB has funds to purchase and install a Barcoding Inventory System in its warehouses. FEDERAL ALN 84.027 AND 84.173 2022-001 Internal Control Over Compliance With Federal Suspension and Debarment Requirements Finding Summary 2 CFR ? 180 requires Independent School District No. 284 (the District) to establish and maintain effective internal control over compliance with requirements applicable to federal program expenditures, including suspension and debarment requirements applicable to the special education cluster federal program. The District did not have sufficient controls in place within its special education cluster federal program to assure that it was not contracting for goods or services with parties that are suspended or debarred, or whose principals are suspended or debarred from participating in contracts involving the expenditures of federal program funds. Corrective Action Plan Actions Planned ?
