Student Financial Assistance Cluster – Federal Assistance Listing Numbers 84.007, 84.033, 84.038, 84.063, 84.268 Recommendation: We recommend the College evaluate its policies and procedures to ensure there is a process in place to ensure timely refund of Title IV credit balances. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: The Student Accounts Office has revised the procedures surrounding all student balances. Balances will be evaluated once a week and a refund will be issued in the next check run. Previously, the Office was conducting this evaluation for the first few weeks of the semester or when a special case occurred. In addition, an error in the report excluded certain balances. The Office has now revised the report to include all students. We observed the following condition in connection with our testing of the various U.S. Department of Education, Title IV, Student Financial Assistance Programs: • Per 34 CFR 668.34, one (1) student out of 15 tested for satisfactory academic progress requirements (SAP) received Title IV, HEA program funds in the amount of $6,342 and was not meeting the requirements specified by the University. The University subsequently returned the funds. The University should implement corrective actions to ensure that the above findings are resolved and do not recur in future periods. Corrective Action – The University agrees with the finding. To address, the University’s registrar’s office will flag students in the student information system and place a registration hold on their account if they are not currently meeting Satisfactory Academic Progress (SAP) requirements. The financial aid office will check for all holds, any former SAP corrective actions and ensure that all students, including those re-entering the University following an absence, are meeting SAP requirements.

Finding: During our audit, we noted that certain figures used as inputs to the annual performance report could not be reconciled to supporting documentation and therefore, we were unable to substantiate certain amounts reported to NYSED. The review of the annual performance report was not performed at an appropriate level of precision such that the incorrect and/or incomplete information presented would be identified and corrected prior to submission to NYSED. Recommendation: We recommend that the District reevaluate the system of internal control for the review and approval of the annual performance report prior to submission to NYSED, including the reconciliation of amounts included within the support to appropriate supporting documentation. District Response: The District will ensure that, prior to submission to NYSED the annual performance report will be reviewed by an individual other than the preparer and reconciled to the supporting documentation in order to confirm the completeness and accuracy of information reported. In addition, all FS10-F reports (Final Expenditure Reports) were submitted, in compliance and approved by NYSED Grants Finance. Furthermore, our Desk audit was completed and approved by NYSED on October 3, 2024. Mr. Salvatore Carambia, Business Administrator, is the person responsible for the planned corrective action. The completion date for this action is November 30th, 2024.

Finding 2024-001 – U.S. Department of Education, Title III, Higher Education, Strengthening Historically Black Colleges and University Programs: During our testing of time and effort reporting, we noted some time and effort reports were incomplete and attached human resource transaction forms did not identify budget/percent allocation for grant funding. The University did subsequently provide corrected time and effort reports after the error was identified during the audit. Auditor's Recommendation – The University should implement corrective actions to ensure that the above findings are resolved and will not recur in future periods. Corrective Action – We concur with the auditor’s finding regarding the completion of our time and effort forms. The time and effort forms were corrected in a timely manner. All time and effort forms are due to the principal investigator by the third day of the subsequent month. We have since developed time and effort instructions and have distributed the instructions to the managers/supervisors of grants funded faculty and staff. Additionally, time and effort instructions will be included in our Human Resource orientations and as well as be distributed during our Faculty and Staff Institute.

CORRECTIVE ACTION PLAN October 21, 2024 Berkeley County Public Service Water District respectfully submits the following corrective action plan for the year ended June 30, 2024. Name and address of independent public accounting firm: Brown, Edwards & Company, L.L.P. 1909 Financial Drive Harrisonburg, VA 2280l Audit period: June 30, 2024 The findings from the June 30, 2024 Schedule of Findings and Questioned Costs (the "Schedule”) are discussed below. The findings are numbered consistently with the number assigned in the Schedule. FINDINGS- FINANCIAL STATEMENT AUDIT 2024-001: Material Audit Adjustments (Material Weakness) Condition: During the audit, we detected material misstatements in the trial balance. Generally accepted auditing standards dictate that detection of errors in an audit is a strong indicator of a significant deficiency or material weakness. Accordingly, we are required to communicate this finding as such. Recommendation: Material audit adjustments indicate that financial information presented to us for the audit was missing or inaccurate. We recommend that management implement processes to ensure accuracy of a accounts. Additionally, all adjustments that were made as a result of our current year audit should be reviewed during the next year as a reminder of matters needing accounting attention in preparing for the 2025 audit. Corrective Action: The District uses outside parties to oversee grant management and lease calculations, both items that required material adjustments. District management will review work performed by outside parties to ensure completeness and accuracy. FINDINGS AND QUESTIONED COSTS-MAJOR FEDERAL AWARD PROGRAM AUDIT 2024-002: Single Audit Performance -Assistance Listing #66.468 and Reporting Condition: A single audit was not performed for a major program for the fiscal year ended June 30, 2023. Criteria: A single audit in accordance with the requirements set forth in the Uniform Guidance is required if total federal expenditures exceed $750,000 in a fiscal year. Federal expenditures exceeded $750,000 and the major program was a high-risk Type A program for the year ended June 30, 2023. Cause: The program required revolving loan fund drawdowns, which did not occur within the fiscal year funds were expended. Effect: The identified Type A high risk program was not tested as major. Questioned Costs: N/A Recommendation: Ensure management considers federal award compliance requirement and ensures that such requirements are satisfied each year. Corrective Action: Management will monitor major programs and ensure that they are tested when necessary. The grant in question was tested during 2024. 2024-003: Controls Over Cutoff - COVID-19 Coronavirus State and Local Fiscal Recovery Funds - Assistance Listing #21.027 and Compliance- Material Weakness Condition: During our review of CSLFRF expenditures, we noted approximately $2,577,622 of allowable costs that were recorded in the wrong period. Criteria: The expenditures must be reported in the proper period for accurate reporting on the Schedule of Expenditures of Federal Awards. Cause: Procedures in place to ensure all expenditures are recorded in the proper period were not followed. Effect: Approximately $2,577,622 of allowable costs were recorded in fiscal year 2025 instead of fiscal year 2024. Questioned Costs: N/ A - the expenditures in question are allowable costs that were reported in the wrong fiscal year. Perspective Information: Five invoices were recorded in the wrong fiscal year. Recommendation: We recommend continued communications with all individuals involved in the grant process to ensure activity is recorded in the proper reporting period. Corrective Action: The District uses an outside party to oversee grant management. District management will review work performed by outside parties to ensure completeness and accuracy. Classical Charter Schools will take the necessary steps to comply with the cash basis reporting method on all future Annual Education Stabilization ESSER Funds reports. Pittsburgh Institute of Mortuary Science Student Financial Aid Audit Corrective Action Plan This corrective action plan is in response to the institute's single audit report for the year ended June 30, 2024, prepared by McClintock & Associates, P.C. Finding 2024-001: Pell Grant Award Recommendation: The Institution should adjust the 2023-2024 Pell awards according to the amounts listed in Summary. Corrective Action: The Institute has returned the Pell awards in the amounts listed in the Summary. The Institute wrote off the dollar amounts so as not to penalize the students identified. The Institute has also reviewed all students who received a Pell award du1ing the current Fiscal Year and recalculated and returned funds appropriately. The Institute again wrote off these dollar amounts. The Institute now has a clear understanding as to how and when the Pell awards should be recalculated. Since this item directly applies to our online student population only, the Institute has rewritten and implemented a specific "Pell Recalculation for Online Students" portion into the already robust Withdrawal procedure. Additionally, the Institute has initiated a weekly meeting to review all students who changed statuses during the prior week. Any online, Pell-eligible students will be recalculated during the meeting and funds will be returned within 7 days of the meeting when deemed necessary. Persons Responsible for Corrective Action: Financial Aid Advisor, Senior Financial Aid Administrator, and Registrar/Director of Administrative & Student Services. Finding 2024‐001 – Special Tests and Provisions – Significant Deficiency in Internal Control over Compliance It was identified that the implementation of a new electronic health record system significantly impacted the control environment as it relates to compliance with federal awards. As a result, the control system did not consistently apply the appropriate sliding fee discounts for patients based on qualification criteria and certain patients were billed amounts less than the sliding fee discount schedule. To address the finding related to system limitations that resulted in patients being billed amounts lower than those specified in the sliding fee discount schedule, the Organization will implement a comprehensive corrective action plan. First, a thorough review of the electronic health record system will be conducted to identify specific limitations as it relates to data capture of federal poverty level at time of patient registration. This has already been completed. Second, the Organization will modify the set-up of its electronic health record to systematically seek approved and active documentation related to sliding fee discounts and will modify the system to not allow sliding fee discounts without the presence of said documentation. The Organization is in the process of setting a meeting to modify its system build, which it expects to be finalized in 30 calendar days. To ensure ongoing compliance, regular monitoring procedures will be established to review data capture accuracy, with the Chief Financial Officer overseeing this effort and the first review scheduled for 30 calendar days from the date on which the electronic health record system fix is implemented. Finally, thorough documentation of all corrective actions taken, including system changes and monitoring results, will be maintained. The Chief Financial Officer will report findings to management on a monthly basis, with the first report due one month from date on which the system fix is implemented. Through these measures, the Organization aims to enhance billing accuracy, ensure compliance with federal requirements, and prevent future discrepancies.

The issues identified in the previous audit were identified part way through fiscal year 2023. Corrective actions identified in the prior audit and listed below are in place going forward. To address these issues, SBU employees have taken the following corrective measures: 1. We reworked the reporting process for withdrawals. All withdrawals now go to the Associate Provost regardless of campus or program. They are then processed by the Registrar's Office and placed in a shared drive. Once there, they are reviewed weekly by the Financial Aid Office, and R2T4s are completed in a timely manner. If there are any withdrawals outside of the normal process timeframe they are escalated and the Registrar and Executive Director of Financial Aid are notified. 2. R2T4 requests are completed by one Financial Aid staff member and verified and processed by another to ensure accuracy and reliability. 3. We have implemented an administrative withdrawal process to give campus and program directors the ability and authority to withdraw students who are no longer in attendance to limit the number of all Fs at the end of the semester.

Lakeshore Community Health Care, Inc. (the "Organization") submits the following corrective action plan for the identified finding and questioned costs for the year ending May 31, 2024. Finding 2024-001: Sliding Fees Statement of Condition: External auditors reviewed 40 sliding fee transactions to verify if the amount charged under the Organization's sliding fee program was calculated properly based on the patient's income level and in compliance with the Organization's sliding fee policy. External auditors noted that one of the sampled transactions was not properly determined resulting in the patient being undercharged for services. Corrective Action: The Organization will review its sliding fee policies and monitor determinations. Staff responsible for sliding fee determinations will be trained on properly computing sliding fees. The Organization will also review the Organization's electronic health record system's controls and safeguards for assisting in sliding fee calculation accuracy. 2024-003 Finding: Special Tests - Wage Rate Requirements Federal Assistance Listing Number 84.425U - COVID-19 - Elementary and Secondary School Emergency Relief Fund (ESSER III) Passed-through Colorado Department of Education Award Number - 4414/4431/9414; Award Year 2021 Summary of Finding: The District did not have adequate internal controls in place to ensure that all certified Payrolls were obtained or reviewed for both the contractor and subcontractor, so laborers and mechanics employed by contractors or subcontractors may not have been paid prevailing wage rates. Status: Corrective action in progress Client Planned Action: The District concurs with the recommendations and is currently developing and implementing internal controls to ensure Compliance. Grants Dept. personnel met with Capital Construction and Procurement Personnel to discuss the processes and procedures to implement, and internal controls that would ensure this. The District’s Grants Department will: 1. Require departments/teams utilizing federally funded grants which involve construction/labor, to designate two staff members responsible for collection of wage-rate payroll certifications. 2. 2024-002 Finding: Allowable Costs and Allowable Activities Federal Assistance Listing Number 84.425U - COVID-19 - Elementary and Secondary School Emergency Relief Fund (ESSER III) Passed-through Colorado Department of Education Award Number - 4414/4431/9414; Award Year 2021 Summary of Finding: The District did not have adequate internal controls in place over the ESSER grant which resulted in unallowable costs being applied to the grant and inconsistently applying indirect costs to the grant. Status: Corrective action in progress Client Planned Action: The District concurs with the recommendations and is currently developing and implementing internal controls to ensure compliance. The inadequate internal controls that caused the inconsistency in supporting payroll information involved the End-of-Year Closeout process. The District will ensure End-of-Year Closeout procedures are up to date and adhered to. These procedures will include a second review of calculations used to determine the expenditure amount in accruals, to ensure it recalculates. The District will also conduct a second review of the supporting detail used to determine Indirect Costs to ensure they are consistent with CDE recommendations and District policies and procedures. Going forward enCircle will also require that all gift cards can be uniquely identified with a specific child and that also foster parents will submit receipts to enCircle regarding gift card purchases until the card is fully spent (if the card is not fully spent the foster parent will be liable to return it or the cash value remaining). enCircle will develop and implement an internal auditing procedure and cycle to regularly evaluate a sample of transactions throughout the year to ensure documentation and use is appropriate for all federal funds.

During the monthly billing process, enCircle will now only bill up until the approved budget even if there are allowed costs, irrespective of budget, in excess of budget amounts. enCircle will then request a budget amendment to allow for these costs and once approved include the previously unbilled costs in the next monthly billing. Furthermore, enCircle will work to preemptively request budget amendments by forecasting allowed expenditures. enCircle will evaluate if the monthly meeting between grant management personnel and financial personnel remains sufficient to ensure communication and grant compliance are adequate. If not, enCircle will change the meeting cycle to create sufficient communication including other means (Teams chats, etc…)
