During our testing, we noted that the Organization does not maintain formal pay rate verification documentation, such as a salary authorization form or personnel action form, supporting changes to the Executive Director’s rate of pay.

Corrective Action Plan

AAFS will be utilizing a formal Pay Rate Authorization Form to document the board approval of any future pay rate increases for the Executive Directors. Moving forward, all board approved pay rate increases will be documented using this form.
