A. Comments on Findings and Recommendations: 2022-001 - Missing Proof of Loan Entrance Counseling. It seems that the student may not have completed entrance counseling. When this student started, MCU was contracted with Weber as its Third- Party Servicer who used to check entrance counseling before processing our students. Since 2020, MCU has been contracted with Campus Ivy whose platform now requires the Financial Aid Department to upload the entrance counseling proof before processing can occur. B. Actions Taken or Planned: 2022-001 - Missing Proof of Loan Entrance Counseling. The student in question has now performed the required Entrance Counseling. Since May 2020, MCU's updated entrance counseling process with Campus Ivy has helped mitigate a risk of gaps with regard to the completion of entrance counseling. MCU will perform an internal review on current students enrolled before May 2020 to ensure entrance counselings are complete.

A. Comments on Findings and Recommendations: 2022-003 - Untimely Enrollment Status Reporting. The Financial Aid Department has a consistent procedure surrounding NSLDS updates, but like many schools, encountered some technical issues over the summer when the NSLDS platform was updated. B. Actions Taken or Planned: 2022-003 - Untimely Enrollment Status Reporting. MCU switched over to Campus Ivy performing its NSLDS reporting in December 2022 which helps eliminate the duplication of efforts in updating CORE and NSLDS. This should also help to close any potential gaps in reporting.

A. Comments on Findings and Recommendations: 2022-002 - Incorrect Refund Calculations. This seems like a simple administrative error surrounding the break and that the break should have been included. The Financial Aid Operations Administrator was still in their final R2T4 training phase at this point in 2022, having taken on the task during the prior year. B. Actions Taken or Planned: 2022-002 - Incorrect Refund Calculations. The Financial Aid Department has updated their internal procedures for R2T4's to make them even more robust, adding further emphasis on the scheduled trimester break section within its R2T4 template. This should help further mitigate the risk of mix up when performing a few at the same time. MCU will refund the resulting overage to the student.

Finding No. 2022-004 Significant Deficiency Personnel Responsible for Corrective Action: Teri Gregory, CFO of Child Center~Marygrove Anticipated Completion Date: March 31, 2023 Corrective Action Plan: Marygrove CFO will create electronic folders on our system that include subfolders for each report filed. The subfolder will contain all reports and correspondences used to create the required filing. Once the filing is created it will be forwarded to the CEO or the CFOO of Catholic Charities (CFOO) for review prior to submission. Once the CEO or CFOO approves the report, the filing will be finalized in the PRF Reporting Portal. A copy of the final report and copies of all emails related to the review will be retained in the corresponding subfolder.

The Auditor discovered that two weekly reports for September 2021 lacked documentation. Compared to the number of tests reported overall in 2021-2022, the number of unverified tests constitute a finding for non-compliance and indicative of issues surrounding record keeping. It has since come to light that some tests were inconclusive but were not identified as such leading to a higher test count versus negative/positive counts. No program specific corrective action steps shall be instituted as this is not an on-going program. However, it will be important to laboriously work out the details prior to an agreement and specify the need for clerical support in future agreements. In addition, the district will review its records and verify alignment with reports submitted to the Los Angeles COE.

Finding 2022-008 ? Cash Management ? Untimely Disbursements During the audit, it was noted that Student Aid Portion grant funds were not disbursed within 15 calendar days of the drawdown from G5. The Institution agrees with the finding. The Institute agrees with this finding, the funds were disbursed later than 15 days after drawdown of the funds. The school was aware that the funds were not disbursed in a timely manner due to timing issues within the department that was responsible to release the funds. In the future, the school will better prepare the checks and letters, so that the drawdown will be completed once the school is ready to release the funds.

Finding 2022-001 ? Underawarded Pell Grants: During the audit, it was noted that two students were not awarded the full amount of their allowable Pell grants. It was recommended that the school credit the appropriate students? accounts $1,077. The Institution agrees with the finding. The school has made the appropriate corrections to the student accounts and both students have received their additional funding. The Institute recognized that our 3rd party Servicer was not disbursing the correct amount of Pell grant funds and requested that they complete a full audit of disbursements, the error on their end has been corrected and should not lead to additional findings moving forward.

The Institute acknowledges that the lag time between registration and financial aid did contribute to this issue. Similar to the resolution above, the director will continue to monitor these issues and work between the financial aid and business offices to ensure that refunds are made in a timely manner.

Finding 2022-002 ? Incorrect Refund Calculation: During the audit, one student had an incorrect refund calculation resulting in $1,592 that should be returned to the Department of Education. The Institution agrees with the finding. The erroneous action happened due to administrative oversight, the refunds to the Department have been completed in the amount of $211.00 Pell grant and $1,381 in Subsidized Direct loan funds. The school understands the importance of calculating the Title IV refund correctly, as a new financial aid administrator and director move into these roles, more oversight from the director position will be initiated.

Finding 2022-004-- Inaccurate Program Data to NSLDS Management Response: Beloit College?s IT and Registrar?s Office identified the issue in the software system causing the incorrect dates to populate and are working to correct it. Because the Registrar pulls the program information out of the software system, the correct information will be provided as soon as the software issue is remedied. After the software issue is fixed, the Financial Aid Office will audit program level data for accuracy no less than once per semester. The Registrar and Financial Aid Office share a report to process mid-semester withdrawals. An additional column was added to this shared report to more clearly display the date the Registrar should be reporting to the NSLDS when a student withdraws mid-semester. Furthermore, the Financial Aid Office will audit the effective dates reported for mid-semester withdrawals to verify the Registrar is reporting the correct dates. During our fiscal year 2022 audit, the period one provider relief fund (PRF) report was tested. An error was discovered in the report involving the final Medicaid cost report adjustment. While this adjustment was included in the PRF report, the entry made to include it was made in the exact opposite direction from how it should have been recorded. This resulted in overstated lost revenue. Upon correcting the entry and balancing it against the applicable fiscal year, the clinic was still able to fully allocate the lost revenue against the PRF funds. After the corrections were made, there was still $30,128 in lost revenue that was not covered by the funds. In an effort to correct this error with HHS, Angela Gargus, CFO, called the PRF provider support line. She explained the situation and asked for the portal to be opened so she could update her report. She talked with at least three different levels of support. While all understood her desire to update the information, she was told since HHS was already up to period 4 and 5 that the likelihood of being able to submit a correction was slim. The support line did take her official request with the details of the correction so a formal response could be obtained for the clinic?s files. Her official request was denied by HRSA PRB Inquiries on January 4, 2023. The denial stated the report was closed and changes could no longer be made. She was instructed to maintain all records related to the organization?s PRF payment for three years. At this time, all actions that can be taken to correct this error have been tried. Ms. Gargus has stored the corrected report as well as the documented attempt to submit the corrected file to HHS in the file with the original PRF submission. The clinic?s CEO and Board of Directors are aware of the error and the actions taken to attempt to fix it.

As stated with the first finding The District went through a change in Business Managers and additional guidance was needed. The Business Office continues to enhance year-end closing procedures with the intent of accurately capturing the District?s financial position and activity for the fiscal year end prior to the audit engagement. The district will enhance the procedures with the preparation of the Schedule of Expenditures of Federal Awards to ensure completion in a timely manner. The District will continue to utilize its resources throughout the fiscal year to minimize audit adjustments required. 2022-001 Community Development Block Grants/State?s Program and Non-Entitlement Grants in Hawaii We recommend that the City only report actual program income expended during the fiscal year instead of amounts authorized to be expended. Management?s Response: We concur with the finding. The City currently has a large grant from CDBG related to the upgrade of the Memorial Park Pool Facility. The City inadvertently reported a use of program income towards the project (which is required). However, it was determined that the grant funds were first used as the project was still ongoing. Finding: In the report submitted to the Health Resources & Services Administration (HRSA) PFR Reporting Portal for reporting Period 1 for Southern Illinois Hospital Services (SIHS), the Corporation indicated that SIHS is the parents of Southern Illinois Medical Services (SIMS), and that SIHS is reporting on SIM's general distribution payments. The SIHS PFR report for Period 1 included the revenue form SIMS in the lost revenue calculations. SIMS also submitted a report to HRSA in the PFR portal for Period 1 targeted distributions under SIMS's TIN. The SIMS lost revenue calculation included the same SIMS revenue that was reported by SIHS. Corrective Actions Taken or Planned: Name of person responsible for corrective action: Warren Ladner Title: Vice President/CFO/Treasurer. There will be a review process put into place in which 2 individuals will be involved in the collection and submission of data into the PRF portal. The review will include all back-up files used for summarizing the data as well as source documents as applicable. As the final step, once data is input into the portal by the person responsible for submission, it will be saved and put into format so that the separate reviewer can verify its accuracy prior to final submission to HRSA. Expected completion date: The corrective action plan is expected to be completed by September 30, 2022.
