CSN – Agrees with the finding. • Detailed corrective action taken, including what will be done to avoid the identified issues in the future, and when these measures will be in place: The Office of Grants and Contracts Post-Award Management assigned a Senior Accountant to identify instructional or non-instructional expenditures when billing the sponsor monthly. This analysis assists the project director of this award to monitor the percentages throughout the year. CSN may adjust the spending to ensure that administrative non-instructional costs will not exceed the allowable amount. CSN will also budget for less than the maximum proportional limit negotiated for non-instruction. Therefore, if instruction costs are less than expected, we will not exceed the non-instructional percentage. This change will be processed with the first budget revision request in FY25. • How compliance and performance will be measured and documented for future audit, management and performance review: Reconciliation of budget to actual expenditures is performed on a regular basis to ensure spending on non-instructional costs is within the maximum limit negotiated with the State eligible agency. • Who will be responsible and may be held accountable in the future if repeat or similar observations are noted: The Project Director of the Adult Education – Basic Grants to States Award will be responsible for repeat or similar observations. WNC – Agrees with the finding. • Detailed corrective action taken, including what will be done to avoid the identified issues in the future, and when these measures will be in place: Western Nevada College (WNC) Adult Education acknowledges the finding related to non-instructional expenditures exceeding the maximum proportional limit as negotiated with the state eligible agency. We recognize the importance of complying with Title 2 of the U.S. Code of Federal Regulations (CFR) Part 200, the Uniform Guidance, and the Workforce Innovation and Opportunity Act (WIOA) requirements. The corrective actions to be taken include the following activities listed below. Review and Alignment of Monthly Reimbursement Requests (RFFs): The Adult Education department will carefully review the monthly RFFs to ensure they align with the approved budget and spending guidelines. A detailed review of these requests will help prevent potential misclassification of expenditures. As part of our corrective action, we will improve this process by verifying that each submission corresponds correctly to the budget categories. Training for Accurate Expense Categorization: We also recognize that a contributing factor to the discrepancies was a lack of clarity on what constitutes AEFLA instructional versus non-instructional costs. This confusion is compounded by the fact that NDE function codes do not always align perfectly with instructional and non-instructional classifications. To address this, we will provide additional training for staff involved in processing grant-related expenses to ensure they are familiar with these distinctions. Enhanced Documentation for Financial Transactions: Moving forward, we will implement a new process in which every grant-related transaction will include specific details in the memo line, such as the grant category, object code, function code, and whether the expense is instructional or non-instructional. This will provide clear guidance to the Controller’s Office team or any future staff processing these transactions, ensuring that expenditures are correctly classified in the general ledger and that we remain in compliance with the allowable limits for instructional and non-instructional costs. Timeline for Corrective Action: We will fully implement these corrective actions by December 31, 2024, and will continue working closely with NSHE and NDE to ensure compliance with federal guidelines and alignment with system-wide practices. • How compliance and performance will be measured and documented for future audit, management and performance review: Monthly general ledger activity and reconciliations will provide documentation that expenditures are in compliance with allowable amounts. • Who will be responsible and may be held accountable in the future if repeat or similar observations are noted: The Adult Education department may be held accountable in the futures if repeat or similar observations are noted. Condition: During our review of ESSER funds, we noted that we could not substantiate all of the pay rates for dollars paid out of the ESSER funds. Criteria: When federal dollars are used to pay for staff payroll expenditures, the rate of pay and/or related calculations of rate, should be documented. This can be in the form of an agreement, salary schedule, or other memo format. Cause: The District did not keep documentation of pay rates for staff who were paid out of ESSER funds. Effect: Staff may not be paid the correct rate. Perspective: The District should have documentation of all pay rates for services provided and for positions. To ensure everyone knows what pay is to be expected. Recommendation: We recommend the District go through and update (or establish) pay rates for all positions and supplemental contracts. Description of Corrective Action Plan: The food service director will pull the End of Day Summary Reports from Lunchtime and input the information into the Child Nutrition Portal. All reports will be provided to the controller to confirm accuracy. Once reviewed and approved, the food service director will submit the report through the Child Nutrition Portal. Auditor Description of Condition and Effect: One of the contracts selected for testing that was subject to the Wage Rate Requirements did not include the required provision, and the District did not obtain the required certified payrolls. In addition, another contract selected for testing had the Wage Rate Requirements required provision, however, the District was unable to obtain the required certified payrolls. Auditor Recommendation: We recommend that the District reviews its policies to ensure that applicable prevailing wage requirements are included in construction contracts whenever federal funds are used and certified payrolls are obtained. Corrective Action: District officials will ensure that construction contracts contain these requirements during the bidding and/or proposal process. Inaccurate Packaging of Federal Direct Loans (FDL) Planned Corrective Action: Shorter University will provide additional training to Financial Aid Staff regarding the importance of reviewing students financial aid offers after manual adjustments are made. The training will include practice scenarios. In addition to training staff, Shorter University's Director of Information Technology is creating a quality control report that will identify students who may be eligible for a subsidized Stafford loan but have not received one. The report will be monitored by the Assistant Director of Financial Aid Systems who will review the students' financial aid. The quality control report will ensure the proper subsidized and unsubsidized Stafford loan allocation. A manual adjustment error was made to a student's financial aid offer, after financial aid was accepted resulting in an under awarding of a Stafford loan. A $3,500 unsubsidized loan to subsidized loan swap will be completed to hold the student harm less and correct the manual adjustment error. Item # 2024-02 Indirect Costs Incorrectly Allocated to Federal Award (Significant Deficiency in Internal Control over Federal Major Program) Criteria: Under Uniform Guidance regulations and per the terms of the federal award, the de minimis 10% indirect cost rate for indirect cost allocations must be used on federal award expenditures. The Guidance also prohibits application of 10% de minimis rate on all subgrants in excess of $25,000 during the period of performance. Condition: Based on the results of our audit testing, we noted indirect costs were allocated incorrectly during the grant period. The total known questioned costs are $1,142. Cause: Management failed to charge indirect costs correctly on the federal subaward during the year ended June 30, 2024. Effect: The effect of the condition was $1,142 in known questioned costs charged to two federal subawards during the year ended June 30, 2024. Auditor’s Recommendation: Management should perform a thorough analysis of the indirect cost allocation to ensure it is reasonable and calculated correctly in accordance with the Uniform Guidance Regulation. Views of Responsible Officials and Planned Corrective Actions: Management understands that indirect expenses incurred on federal awards must be reviewed and allocated appropriately. The financial aid office has continued with the implementation of the calendar that displays the attendance days from the first day of school to the last day of school, referring to the school's master calendar. The financial aid office added an extra verification step of written notification from the Registrar's office of beginning and end days for each semester. 2024-003 Initial Fiscal Year End, 2024 Summary of Finding- During the audit, it was noted that the University used the incorrect number of total days in the payment period or period of enrollment in calculating the percentage of payment period and/or period of enrollment completed. The financial aid office continued with the calendar process showing days of attendance from the first day of school to the last using the school's master calendar as a reference. This will be used also as a double check of days when calculating returns. The dates used in the return calculations were off a day due to misreading the ending date of semester. The Financial Aid Administrator verified the beginning and last day of each semester with the Registrar's office in writing. Anticipated Completion Date- July 1, 2025

Condition: During our review of Food Service funds, we noted that the numbers of meals served was less than the amount that was submitted for reimbursement. Criteria: When a recipient of federal food service money goes to submit for reimbursement, the amount of meals that are served should match up with the reporting out of the recipients system to ensure proper reimbursement is taking place. Cause: The Food service department did not have a proper reconciliation practices to ensure accurate reporting Effect: Funds may be overdrawn. Perspective: The majority of the buildings were accurate as we found during testing, with high turnover in the space training may not have taken place. Recommendation: We recommend training of all food service recording staff as well as the Director of Food service understand that the reimbursement of meals report should match the meals served reporting that documents each day of the meals served. It should also be noted that the meals served documents needs to have oversight to ensure those reports are accurate on a daily basis. Views of Responsible Officials and Planned Corrective Actions: Haven USD 312 staff involved are undergoing training to learn requirements. Processes and procedures are being developed to ensure proper record keeping and reporting.

Student Financial Assistance Cluster – Federal Assistance Listing Numbers 84.007, 84.033, 84.038, 84.063, 84.268 Recommendation: We recommend the College evaluate its policies and procedures to ensure there is a process in place to ensure timely refund of Title IV credit balances. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: The Student Accounts Office has revised the procedures surrounding all student balances. Balances will be evaluated once a week and a refund will be issued in the next check run. Previously, the Office was conducting this evaluation for the first few weeks of the semester or when a special case occurred. In addition, an error in the report excluded certain balances. The Office has now revised the report to include all students. Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: Grant Accounting and Purchasing will both review requisitions within Jaggaer to make sure appropriate bids, and or exemptions are documented or attached.
