Criteria: The District is required to report the number of eligible meals served for reimbursement on a monthly basis. Cause: The District employee responsible for reporting was not adequately trained on the reports generated from the computer software and which meals qualified for reimbursement. Effect: The District did not receive the proper amount of reimbursement for the meals that qualified. Recommendation: Training on the reports generated for the computer software and what meals qualify for reimbursement needs to be completed for those responsible for reporting. The District should also add a review procedure of the reporting prior to submission of a claim for reimbursement. Response: The District will provide additional training for those involved in the reporting process, and will also add an additional level of review prior to submitting claims for reimbursement. Conclusion: Response accepted.
