Corrective Action Plan:
Urban Strategies, Inc. acknowledges the lack of record retention after an employee left
the organization. After a thorough review of allowable costs, all were deemed materially correct.
Urban Strategies, Inc. communicated to the departments involved the necessary improvements to the internal controls that were agreed upon in order to prevent the deficiencies from occurring in the future.
Urban Strategies, Inc. is refining procedures to ensure all reviews and communications are performed, reviewed and documented timely and accurately, as well as ensuring all review documentation is properly retained.
