Finding Text

FINDING 2025-011 Subject: Gaining Early Awareness and Readiness for Undergraduate Programs - Internal Controls Federal Agency: Department of Education Federal Program: Gaining Early Awareness and Readiness for Undergraduate Programs Assistance Listings Number: 84.334 Federal Award Number and Year (or Other Identifying Number): P334A210080 Compliance Requirements: Cash Management, Period of Performance Audit Finding: Material Weakness Condition and Context The School Corporation used reimbursement requests to claim allowable expenses paid from Gaining Early Awareness and Readiness for Undergraduate Programs. These requests must be based on and supported by transactions recorded within the School Corporation's Gaining Early Awareness and Readiness for Undergraduate Programs funds. The School Corporation's internal controls over the Gaining Early Awareness and Readiness for Undergraduate Programs Cash Management and Period of Performance compliance requirements were not consistent throughout the audit period. During our review of all the grant reimbursement requests, it was noted that not all reimbursement requests had evidence of a review. The reimbursement requests submitted from July 1, 2023 through November 30, 2024, were not reviewed and approved. The lack of internal controls was isolated to the period identified above. Criteria 2 CFR 200.303 states in part: "The non-Federal entity must: (a) Establish and maintain effective internal control over the Federal award that provides reasonable assurance that the non-Federal entity is managing the Federal award in compliance with Federal statutes, regulations, and the terms and conditions of the Federal award. These internal controls should be in compliance with guidance in 'Standards for Internal Control in the Federal Government' issued by the Comptroller General of the United States or the 'Internal Control Integrated Framework', issued by the Committee of Sponsoring Organizations of the Treadway Commission (COSO). . . ." Cause Management of the School Corporation had not established a proper system of internal controls over reimbursement requests to ensure compliance with the Cash Management and Period of Performance compliance requirements. INDIANA STATE BOARD OF ACCOUNTS 43 SOUTH BEND COMMUNITY SCHOOL CORPORATION SCHEDULE OF FINDINGS AND QUESTIONED COSTS (Continued) Effect The failure to establish a system of internal controls could have enabled material misstatements or irregularities to remain undetected. The failure to monitor the internal control system placed the School Corporation at risk that internal controls may not be either designed properly or operating effectively to provide reasonable assurance that internal controls would have prevented, or detected and corrected, misstatements in a timely manner. Questioned Costs There were no questioned costs identified. Recommendation We recommended the School Corporation's management implement consistent internal control procedures to ensure all grant reimbursement requests are properly reviewed and approved before submission. This should include establishing a formal review process, requiring documented sign-off for every request, and periodically monitoring compliance throughout the grant period. Strengthening these controls will help ensure adherence to Gaining Early Awareness and Readiness for Undergraduate Programs Cash Management and Period of Performance compliance requirements and improve overall grant oversight.
