We noted in six (6) instances that the addresses on the supporting documentation were not the addresses found on the application.

Corrective Action Plan

It is recommended that Management should verify that all participant files are maintained and contain all required documentation for eligibility. Corrective Action and Explanation - The City of Newark will comply with the Auditor's recommendation. Implementation Date - December 31, 2025
