Financial records must adequately identify the source and application of funds and provide accountability for all funds, property, and other assets related to the Federally-funded activities. Condition: The Office of the State Auditor (OSA) issued finding 2019-021 as a result of procedures performed for the fiscal year 2019 audit. This finding identified that “Program personnel did not take the existing cash balance into consideration when requesting Federal funds for the Food portion of the WIC grant.” This resulted in an excess cash balance for the Food grant. The finding continues to be repeated as the Department has not returned the funds to the Federal awarding agency and the excess cash balance remains. Context: The Department calculated a $1,055,104 residual cash balance from the 2013 WIC Food grant and a $4,100 residual cash balance from the 2018 WIC Food grant. Cause: Lack of adequate recordkeeping and account reconciliation in prior years. The Department has made efforts to seek disposition from the Federal awarding agency; however, the issue has not been resolved. Effect: The State may be required to return $1,059,204 to the Federal awarding agency. Recommendation: We recommend that the Department continue efforts to resolve this matter with the Federal awarding agency. Corrective Action Plan: See F-18 Management’s Response: The Department and the DHHS Financial Service Center agree with this finding. The Department must maintain accurate, current, and complete disclosure of the financial results of each Federal award or program in accordance with reporting requirements. Condition: The Maine Emergency Management Agency (MEMA) administers the Disaster Grants – Public Assistance (DG – PA) program for the State. MEMA is required to submit quarterly DG – PA Federal Financial Reports (FFRs) to the Federal Emergency Management Agency (FEMA) Regional Office. FFRs provide FEMA with the status of funds for the award, Federal expenditures, and cost-sharing requirements. The Office of the State Auditor (OSA) tested four FFRs due in fiscal year 2024 and found: • one FFR inaccurately reported total Federal funds authorized as $889,510 when the correct total was $617,318; • one FFR inaccurately reported the recipient share of expenditures as $712,412 when the correct share was $159,382; and • one FFR inaccurately reported total Federal funds authorized as $640,654 when the correct total was $1,469,390, and the recipient share of expenditures as $842,604 when the correct share was $434,484. OSA selected a non-statistical random sample. Context: During fiscal year 2024, 33 FFRs were required to be filed by MEMA for the DG – PA program. Cause: • Lack of adequate policies and procedures to ensure data used for financial reporting is complete and accurate • Lack of supervisory oversight Effect: • Noncompliance with Federal reporting requirements • Inaccurate tracking of subawards may result in noncompliance with Federal matching requirements. Recommendation: We recommend that MEMA enhance policies and procedures to ensure that FFRs are accurate and include all required information for compliance with Federal reporting requirements. Corrective Action Plan: See F-30 Management’s Response: The Department agrees with this finding. Corrective action was implemented at the end of State Fiscal Year 2024. Untimely reports were primarily due to a backlog of required reporting that was brought up to date over the fiscal year as a corrective action to the prior year finding. In the current monthly reporting process, award data is provided by grant program experts to avoid incorrect data elements, and reporting is reviewed for completeness by a staff member not involved in report submission. In the third quarter of FY2025, the FFATA reporting system was retired from FSRS.gov and transferred to SAM.gov, further minimizing the possibility of incorrect data elements being reported. Finding 2024-001, Expense Allocations - Financial Management (Assistance Listing 16.575) Criteria: Per 2 CFR § 200.302(a) (Financial Management), all recipient and subrecipient financial management systems must be sufficient to track expenditures and establish that funds have been used in accordance with federal statutes, regulations, and the terms and conditions of the federal award. Condition and Context: The Agency’s general ledger for the program included occupancy expenses that were not used for the program. Although these expenses were erroneously recorded in the general ledger under the program, they were correctly excluded from the submitted claims. Cause: These expenses were incorrectly allocated in the general ledger due to weaknesses in internal controls over expense classification and allocation. The financial management system lacked adequate review mechanisms to ensure expenses were properly assigned to the correct program and reconciled with the submitted claims. Effect: The misallocation of expenses in the general ledger resulted in financial reports that do not accurately reflect actual program expenditures. This misstatement could impact budgetary decisions, financial reporting accuracy, and overall compliance with federal regulations. Identification as a Repeat Finding: No. Questioned Costs: None. Recommendation: We recommend that the Agency strengthen its policies and procedures for expense allocation by implementing a more structured review and approval process. This should include enhanced internal controls, periodic reconciliations of expenses between the general ledger and submitted claims, and staff training on accurate cost classification. Additionally, management should establish clear documentation procedures to support the proper allocation of expenses in the general ledger and ensure compliance with federal regulations.

Context: The School Corporation was required to submit two Annual Data Reports to the Indiana Department of Education (IDOE) during the audit period to meet federal reporting requirements. For both reports submitted, we noted there was no formal review of the reports prior to submission to IDOE. Additionally, we noted that the school reported an amount of $0 for ESSER I, II, and III on the annual data reports submitted for the period of July 1, 2021 through June 30, 2022, which did not agree to underlying expenditure records ($16,924, $402,123, and $620,907, respectively) for the same period. The school also could not provide support for the number of full-time employees (FTE) as of 9/30/22 reported on the Year 3 CrossAct report. Further, we noted that the school reported $0 for ESSER II on the Year 3 report covering the period of July 1, 2022 through June 30, 2023, which did not agree to the underlying expenditure records ($248,418). The school also could not provide support for the number of full-time employees (FTE) as of 9/30/23 reported on the Year 4 CrossAct report. Identification as a repeat finding: Yes. Evaluation of Finding Significant Deficiency Criteria Management is responsible for ensuring compliance with the California Water and Wastewater Arrearages Payment Program Guidelines and the Uniform Guidance, including the timely allocation of funds as bill credits within 60 days of receipt. This includes the design, implementation, and maintenance of effective internal controls to ensure that arrearages are allocated in accordance with the program’s requirements. Failure to allocate the Water and Wastewater Arrearages within the required timeframe could result in non-compliance with both state and federal regulations. Recipients of federal awards must maintain accurate records and documentation to support the proper allocation of funds, in line with 2 CFR 200.302, and take corrective actions if allocations are delayed. Condition During the single audit, we found that Wastewater Arrearages claimed were not allocated as bill credits within 60 days of receipt of funds requested in accordance with the California Water and Wastewater Arrearages Payment Program Guidelines. Cause of Condition The delay in allocating the wastewater arrearages within the required 60-day period was due to challenges in the internal controls and monitoring processes that impacted the timely allocation. The tracking systems in place did not fully support the prompt allocation in accordance with the California Water and Wastewater Arrearages Payment Program Guidelines. Additionally, there were delays in ensuring that the 60-day requirement was consistently met. As a result, the allocation did not occur within the required timeframe, leading to a non-compliance issue with both state and federal regulations. Effect or Potential Effect of Condition The delay in allocating the wastewater arrearages within the required 60-day period may lead to non-compliance with both state and federal regulations, which could result in the disallowance of the funds. This could also impact the agency’s eligibility for future funding or support under the program and may result in increased scrutiny during future audits or reviews. Questioned Costs None Context The City applied the funding properly for the purpose of the program, but due to the previously mentioned causes, missed the 60-day timeframe to apply the customer credits. We do not believe this to be indicative of a systemic noncompliance in the program, when taken as a whole, and is likely not to be repeated in future years. During our audit procedures, we noted that the reports did not agree with the accounting and project records. Criteria: 2 CFR 200.302 (a) states that the states’ and other non-Federal entities’ financial management systems, including records documenting compliance with Federal statutes, regulations, and the terms and conditions of the Federal award, must be sufficient to permit the preparation of reports required by general and program-specific terms and conditions; and the tracing of funds to a level of expenditures adequate to establish that such funds have been used according to the Federal statutes, regulations, and the terms and conditions of the Federal award. Cause: The School Corporation's management implemented a review control over the annual data reports, however, it was not sufficient enough to detect and prevent errors in annual data reports submitted to the Indiana Department of Education. Effect: Annual data reports submitted during the audit period to the Indiana Department of Education contained material errors compared to underlying transaction detail for the period reported. Questioned Costs: There were no questioned costs identified. Context: The School Corporation was required to submit Annual Data Reports to the Indiana Department of Education (IDOE) during the audit period to meet federal reporting requirements for ESSER grant awards. We noted that the ESSER I amount reported on the Year 3 report ($266,367) did not agree to the underlying expenditure record ($96,019) for the period of July 1, 2021 through June 30, 2022. Additionally, the ESSER II and ESSER III amount reported on the Year 2 report ($1,433,207, and $643,771, respectively) did not agree to the underlying expenditure records ($1,400,698, and $630,465 respectively) for the period of July 1, 2021 through June 30, 2022. These expenditures were originally expended from the Title I 2021 grant award fund 4121, requested for reimbursement and then the expenditures were moved to other funds. Because these expenditures were reappropriated, they were not an allowable activity or cost of the 2021 Title I grant award. In addition, the School Corporation was unable to provide supporting documentation for $6,646 worth of certified salary expenditures requested for reimbursement for the same grant award from February 17, 2022 to June 30, 2022. It was determined that this amount was double requested for reimbursement and were not an actual expenditure. The total amount of $49,787 was considered questioned costs. Subsequent to the 2021 Title I grant award, the School Corporation established and implemented a process and internal controls to ensure expenditures for the 2022 and 2023 awards from July 1, 2022 through December 31, 2023, were for allowable activities and costs and in conformance with the cost principles. The vendor expenditures are initiated by the Title I Director and the Title I Administrative Assistant. Payroll is reviewed each pay period by the Title I Administrative Assistant. The Business Manager/Treasurer prepares the reimbursement request using a detailed expenditure report from their accounting system. The Title I Administrative Assistant verifies the information entered into the reimbursement request by also comparing it to the detailed expenditure reports. The Title I Administrative Assistant also reconciles the Title I award to the expenditures. If the Title I Administrative Assistant identifies that a correction of errors needs to be made to a Title I fund, they fill out a Corrections Form. The Title I Director then reviews and signs the form and provides it to the Business Manager/Treasurer to make the correction in the accounting system prior to completing a request for reimbursement. After the corrections have been made, the Title I Administrative Assistant verifies the changes were correctly made. After all corrections are made, the reimbursement request is approved by the Title I Director and then submitted by the Business Manager/Treasurer.
