Finding Text

Criteria: In accordance with 2 CFR 200.328, AEA is required to submit performance reports as required by the terms and conditions of the Federal award. These reporting requirements are further described by the pass-through funder to AEA. Condition: During our testing of AEA’s financial and performance reporting requirements, AEA was unable to demonstrate full timely submission of required financial reports and performance reports. Timely compliance submission is required for financial reports and performance reports. For the grants tested, reporting requirements ranged from monthly to quarterly financial and/or performance reports. We noted 18 instances where the financial report was not submitted within the required time frame and 10 instances where the performance report was not filed within the required timeframe. Cause: AEA did not maintain sufficient procedures to ensure required reports were submitted timely and that documentation evidencing submission and review was retained. Effect: Failure to maintain support demonstrating submission of required reports increases the risk that federal awarding agency and pass-through entities may not receive complete, accurate, and timely information necessary to monitor program performance, expenditures, and compliance with Federal award requirements. Questioned Costs: None reported. Context: We tested a sample of financial reports and performance report submitted during the fiscal year and identified 10 out of 20 reports that was submitted after the required due date. Repeat Finding: Not applicable. Recommendation: We recommend that AEA strengthen its reporting controls to ensure all required financial and performance reports are prepared, reviewed, submitted, and retained in accordance with Federal award requirements. Procedures should include maintaining evidence of submission, management review, and monitoring of reporting deadlines. View of Responsible Officials: Refer to management’s corrective action plan.

Corrective Action Plan

Responsible contact(s): Steve, Staff Accountant - AR, for financial reports; Program Directors/Managers for performance reports; Agnes, Controller, for financial review; Laura, CFO, for oversight. Participating staff: Program reporting staff and designated backups; Steve, with support from Sharon, for calendar/evidence support; Trina and AP staff for underlying reconciliations. During part of 2026, AEA did not submit the reports as required by the award on a timely manner, this was corrected for the August 2026 submissions. 1. Create a complete award-reporting calendar. Steve and each Program Manager will inventory executed agreements, amendments, and reporting instructions. The register will show award, report type, reporting period, actual contractual due date, submission method, preparer, reviewer, submitter, backup, and evidence link. Agnes will validate requirements and updates; an organization-wide assumed due date will not replace award terms. 2. Build in lead time. Using the external due date as D, AEA will target source data by D minus 10 business days, drafts by D minus 7, review by D minus 5, and submission by D minus 2. Where the reporting window is shorter, Agnes and the Program Director will set feasible earlier internal milestones without changing the external deadline. 3. Assign substantive review. Steve will prepare and reconcile financial reports to the ledger, approved payroll/allocations, prior cumulative reports, budget categories, and claim adjustments. Agnes will independently approve financial accuracy. Program staff will prepare performance results and explanations, and the Program Director will approve them. Finance and Programs will reconcile financial and performance data where related; authorized certifications will be obtained. 4. Retain proof and escalate risk. The designated submitter, program staff, will send a copy of the submitted report to Steve. Steve will save the email, copy of the submitted report, retain the final approved report, dated review, submission timestamp, portal receipt or transmission evidence, funder acknowledgment when available. Steve will index evidence and follow up on rejections. Missed internal milestones or threatened deadlines will be escalated to Laura within two business days. 5. Resolve prior reporting exceptions. Steve and Program Managers will review 2026, submit any outstanding reports promptly (where applicable), and document unresolved evidence gaps. Late reports will remain classified as late; later submission will not erase the original exception. Anticipated completion Validated complete registers and procedures: October 30, 2026. Historical reporting review: November 30, 2026. Anticipated completion after effectiveness review: January 29, 2027.
