Recommendation – The Organization should review the sliding fee discount policy and schedules to ensure compliance with the Health Center Program Compliance Manual for all services within the scope of the program. The Organization should also consider simplifying the methodology used to calculate sliding fee discounts and related system processes to promote consistent application of discounts and improve ongoing compliance. The Organization should also continue educating personnel on the sliding fee discount program and consider implementing internal audits of sliding fee discounts to monitor and identify specific areas for further improvements or training.

Corrective Action Plan

Management Response and Root Cause Management agrees with the finding. Federal funds were drawn in advance of related cash disbursements, including a larger-than-normal draw on January 28, 2025 in response to uncertainty surrounding the federal funding pause. In addition, draw requests were based on general ledger expenditure activity rather than verified cash disbursements, so unpaid expenditures could be included. Although sufficient allowable expenditures supported the funds by year-end and interest did not exceed the federal retention threshold, the process did not consistently limit requests to immediate cash needs. Corrective Action Plan HealthCore Clinic will implement a documented federal cash draw process that ties every request to paid allowable costs, forecasts only near-term cash needs, prevents unsupported advance draws, independently reviews each request, monitors cash-on-hand and interest, and provides clear escalation and approval requirements for unusual circumstances. HealthCore clinic will review all drawdowns in 2026 to date to ensure an expenditure listing exists to support both the allowability of costs and the timing of drawdowns. HealthCore Clinic will provide grants management training to all finance and grant personnel.
