Finding Text

Finding 2025-001 - USDA Food Distribution and Authorized Signers Condition: During our audit, it was identified that the Organization did not consistently obtain signatures from "approved shoppers" for USDA food products. Additionally, in at least one instance, the individual signing for a USDA distribution was not listed as an authorized signer. These issues indicate a lack of adherence to required procedures for verifying and documenting authorized individuals who pick up USDA food products. Criteria: The Organization is required to maintain an "approved shopper" list for each partner agency, specifying who is authorized to pick up USDA food products. Furthermore, the Organization must ensure that those signing for deliveries are included on the approved list and that their signatures are properly obtained and cross-referenced. Cause: Staff turnover within the Organization led to lapses in following established procedures, resulting in missed steps such as obtaining and verifying signatures against the approved shopper list. Effect: As a result of these lapses, signatures were not consistently collected or compared to the approved shopper list, increasing the risk that unauthorized individuals may have received USDA food products and that proper records of distribution were not maintained. Recommendation: We recommend that the Organization provide comprehensive training to all relevant staff on the correct procedures for obtaining and cross-referencing signatures for USDA food product distribution. This will help ensure compliance with requirements and improve the integrity of the distribution process. Views of Responsible Officials: See attached response.

Corrective Action Plan

Finding 2025-001 - USDA Food Distribution Authorized Signer (Repeat Finding - see FY2024 Finding 2024-002) Condition During testing of agency distributions, four instances were identified in which the individual signing for a USDA order was not listed on the agency's approved shopper list. Conective Action The Organization concurs with this finding. Since this condition recurred despite FY2024 corrective training, the Organization is implementing verifiable, technology-based controls rather than relying on training alone: I. The Organization is building the current Authorized Shopper List directly into its delivery software as a perstop note, so each driver sees the agency's approved signers in real time at the point of delivery rather than relying on a separate printed list or memory. 2. Updated Authorized Shopper Lists for all TEFAP agencies will be collected in person, in writing, from each agency at the Organization's annual All-Agency Conference (held at Second Harvest on Friday, September 25, 2026), ensuring the list loaded into the delivery software reflects current, agency-confirmed signers. 3. The Agency Relations Manager, with support from additional administrative staff added to ensure timely completion, will perform a quarterly spot-check comparing signed delivery invoices to the Authorized Shopper List, with exceptions reported to the Executive Director. 4. All delivery and warehouse staff will receive refresher training on authorized-signer procedures once the updated lists and delivery-software notes are in place, with signed training attestations retained in personnel files. Responsible Party: Agency Relations Manager, with oversight from the Executive Director Target Completion Date: Updated Authorized Shopper Lists: September 25, 2026; delivery-software implementation and staff training: November 30, 2026; quarterly compliance reviews: ongoing thereafter
