AI Summary

  • Core Issue: Misclassification of non-Ryan White B patients as part of program income, leading to non-compliance.
  • Impacted Requirements: Compliance with program income recording and internal controls over classification.
  • Recommended Follow-Up: Investigate the cause of misclassification and implement controls to prevent and monitor errors in patient income classification.

Finding Text

Grantees are required to correctly determine, use, and record program income as a condition of the grant. It was noted that non-Ryan White B patients were being included within the grant's program income during the year. 7 out of 25 patients selected for testing were determined to be mis-classified within the program income of the Ryan White Part B program (RWB). The effect is reportable non-compliance with the program income compliance requirement for this grant, as well as, a material weakness in internal controls over compliance. Patients are being mis-classified within the medical billing process as RWB patients. As a result, program income is being overstated by the additional patients' income. The cause of the mis-classification is being researched. It is recommended that the Organization research the cause of the misclassification and develop a preventative internal control to ensure that patient income is being correctly classified initially. Further, it is our recommendation that the Organization develop a monitoring control to detect and correct when errors have occurred in the RWB classification of patients.

Corrective Action Plan

SHR will correct the identified non-Ryan White Part B (RWB) patients who were incorrectly included in the RWB program income. The corrective action consists of generating a report of all RWB patients and matching this report to a separate report listing patient receipts collected. This process will occur on a monthly basis, with RW program collections being posted as community income. This process will also include reconciling the results with an independent report generated by our external collection agency. This QA procedure will ensure that patients included in the RWB program income report are appropriately classified as RWB patients and are not community clinic patients who are not eligible for inclusion in RWB program income. SHR will continue to monitor the effectiveness of these controls and will make additional adjustments as necessary to ensure that RWB program income is accurately classified, recorded, and reported in accordance with applicable program requirements.
