Federal Reporting and Grant Drawdowns Fiscal year finding initially occurred: 2025 Federal Program: Head Start Cluster Assistance Listing Number: 93.600 Federal Agency: U.S. Department of Health and Human Services Type of Finding: Material Noncompliance CONDITION: Avoyelles Child Development Services, Inc. (Agency) failed to submit the required mid-year SF-425 Federal Financial Report for the Head Start program timely. Additionally, the Agency drew $123,272 in federal grant funds associated with proposed capital expenses prior to obtaining formal HUD approval for the expenses. CRITERIA: Federal grant recipients are required to submit required financial reports timely and comply with grant award terms and conditions governing approval and use of federal grant funds. Proposed capital expenses requiring federal approval must receive approval prior to utilization of federal grant funds. CAUSE: The condition resulted from delayed federal reporting caused by internal computer and system-related issues as well as insufficient procedures to ensure federal approval was obtained prior to drawing federal grant funds. EFFECT: The Agency was not in compliance with federal reporting and grant administration requirements. As a result, 1) required federal financial reporting was submitted late, 2) federal funds were drawn down prior to obtaining required approval, and 3) a material liability due to HHS totaling $123,272 existed at year-end. RECOMMENDATION: Management should implement procedures to ensure 1) all required federal reports are submitted timely, 2) federal reporting systems are adequately monitored and supported, and 3) grant funds are not drawn down prior to obtaining all required federal approvals. VIEWS OF RESPONSIBLE OFFICIALS: Management believes that the organization maintains a comprehensive and effective system of internal controls over financial reporting, compliance, and grant administration. The findings relate to a specific control deficiency identified during the audit which were both impacted by a lack of communications from the funding agency and the 43-day Federal Government shutdown from October 1, 2025 to November 12, 2025, and do not, in management's view, reflect a systemic weakness in the overall control environment. Management acknowledges the circumstances that resulted in the findings and recognizes the opportunity to strengthen certain procedures and documentation practices. The conditions identified were limited in scope and occurred despite the existence of established policies, oversight processes, and monitoring activities designed to promote compliance with applicable federal requirements.
