Condition: Neither of the semi-annual performance reports for the program were submitted by the reporting deadline. Criteria: Semi-annual performance reports must be filed within 30 calendar days of the end of the 6-month reporting period. Cause: The staff responsible for the submission failed to submit by the reporting deadline. Effect: The reporting requirements, as established by the grant agreement, were not properly followed. Questioned Cost Amount: Not applicable. Context: Both of the two performance reports required to be filed during the fiscal year were not submitted by the deadline. Recommendation: We recommend that performance report submission are done in advance of the deadline. Views of Responsible Officials and Planned Corrective Action: Performance reports will be filed in a timely manner to avoid missing the deadline.
