The Organization did not maintain a signed income certification form for one of the tenant files tested during the audit. The records are stored with the outsourced property management company, and the annual signed recertification was not able to be located for a certain tenant.

Corrective Action Plan

The Organization accepts the recommendation of Deming, Malone, Livesay & Ostroff and, accordingly, management will maintain signed income certifications for all tenants in accordance with HUD guidelines to ensure proper documentation is maintained in tenant files.
