FINDING 2025-224 The Department overstated federal grant expenditures on the Schedule of Expenditures of Federal Awards (SEFA) closing package. Type of Finding: Significant Deficiency, SEFA Misstatement Assistance Listing Title: American Rescue Plan Act - Coronavirus State and Local Fiscal Recovery Fund Assistance Listing Number: 21.027 Federal Award Number: 340ARPASLFRF23; 340ARPASLFRF24 Program Year: March 3, 2021 – December 31, 2024 Federal Agency: Department of the Treasury Compliance Requirement: U.S. Code of Federal Regulations (CFR) 200.510(b) Questioned Costs: None Criteria: The U.S. Code of Federal Regulations (CFR), Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards (Uniform Guidance) included in 2 CFR 200.303 requires that a nonfederal entity receiving federal awards establish and maintain internal controls that provide reasonable assurance that the nonfederal entity is managing the federal award in compliance with federal statutes, regulations, and the terms and conditions in the federal award. Additionally, 2 CFR 200.510 requires the State to prepare the SEFA, which must include the total federal awards expended for each individual federal award program. The Office of the State Controller requires agencies to complete the SEFA closing package and uses this information to compile the statewide SEFA. Condition: The Department overstated the federal grant expenditures for the Coronavirus State and Local Fiscal Recovery Fund (CSLFRF) on the SEFA closing package. The total fiscal year 2025 expenditures reported for the CSLFRF were $30,784,715, which incorrectly included $2,761,004 of expenditures from fiscal year 2024. Cause: In fiscal year 2024, the State implemented a new statewide accounting system, Luma. The implementation of Luma caused many procedures to change from those used in the prior system. In March 2024, the Department contacted the Office of the State Treasurer (Treasurer) to make a wire transfer for a $2.5 million land purchase. The Department believed that the Treasurer would make an entry for the transfer or contact the Office of the State Controller (Controller) to make an entry for the transfer as would have occurred in the prior statewide accounting system; however, the entry was not made. The Department recognized that the land sale entry had not been recorded in Luma. Over the course of the next several months, the Department had multiple communications with the Treasurer and the Controller to determine the correct method of recording the transaction. The land purchase occurred in fiscal year 2024; however, the expenditure was not recorded in Luma until fiscal year 2025 and was improperly reported as an expenditure in fiscal year 2025 on its closing SEFA package. The Department staff indicated that they were unaware of a mechanism to retroactively report fiscal year 2024 expenditures. The other expenditures in the overstated amount were expenditure invoices entered into Luma in June 2024 that were not approved and released until July 2025. Effect: The Department originally reported fiscal year 2025 CSLFRF expenditures of $30,784,715. The original amount was overstated by $2,761,004. The correct amount that should have been reported was $28,023,711. This error was not corrected in the statewide SEFA. The $2,761,004 was incorrectly excluded from the fiscal year 2024 SEFA causing an understatement. Recommendation: We recommend that the Department design and implement procedures to accurately record, compile, and document the amounts reported in the SEFA closing package and to retain documentation supporting the amounts reported. Management’s View: The department agrees with the finding and asserts that it was a one-of-a-kind error in the timing of recording of one expenditure transaction. The error went undetected for a few months during the very confusing and difficult Luma transition year. When staff detected the error they believed the expenditure was still required to be reported on a SEFA and the only option available was to report it in FY 2025. Corrective Action: Staff have already completely familiarized themselves with the guidance on “New Requirement for Wire Payment Requests” issued by SCO on October 6, 2025. We are confident that our understanding of these new procedures will prevent any future occurrence. Auditor’s Concluding Remarks: We thank the Department for its cooperation and assistance throughout the audit.

Cause of Condition: The County has not designed and implemented formal internal controls for the reporting of its federal programs as required by federal regulations. Effect of Condition: This condition resulted in the material misstatement of the County’s SEFA. Recommendation: OSAI recommends the County Officials and Department Heads gain an understanding of federal programs awarded to McCurtain County. Internal control procedures should be designed and implemented to ensure accurate reporting of expenditures on the SEFA and to ensure compliance with federal requirements. Management Response: Chairman Board of County Commissioners: The current Board of County Commissioners acknowledges the finding. The Board is committed to improving the identification, tracking, and reporting of federal awards. County officials will continue working with auditors and department heads to strengthen reconciliation and review procedures and ensure federal activity is accurately reported on the Schedule of Expenditures of Federal Awards. County Clerk: The County Clerk will work with the Board of County Commissioners to adopt policies and procedures for federal awards. County Treasurer: The County Treasurer acknowledges the finding. The Treasurer's Office will continue maintaining accurate accounting records of federal receipts and expenditures and will cooperate with county officials and auditors during the preparation of the Schedule of Expenditures of Federal Awards. Additional review and reconciliation procedures will be encouraged to improve the accuracy of federal reporting.Criteria: 2 CFR § 200.303(a) Internal Controls reads as follows: The non-Federal entity must: Establish and maintain effective internal control over the Federal award that provides reasonable assurance that the non-Federal entity is managing the Federal award in compliance with Federal statutes, regulations, and the terms and conditions of the Federal award. These internal controls should be in compliance with guidance in “Standards for Internal Control in the Federal Government” issued by the Comptroller General of the United States or the “Internal Control Integrated Framework”, issued by the Committee of Sponsoring Organizations of the Treadway Commission (COSO). 2 CFR § 200.508(b) Auditee responsibilities reads as follows: The auditee must: Prepare appropriate financial statements, including the schedule of expenditures of Federal awards in accordance with §200.510 Financial statements. 2 CFR § 200.510(b) Financial statements reads, in part, as follows: Schedule of expenditures of Federal awards. The auditee must also prepare a schedule of expenditures of Federal awards for the period covered by the auditee's financial statements which must include the total Federal awards expended as determined in accordance with §200.502 Basis for determining Federal awards expended. Further, GAO Standards – Section 2 – Objectives of an Entity – OV2.23 states in part: Compliance Objectives Management conducts activities in accordance with applicable laws and regulations. As part of specifying compliance objectives, the entity determines which laws and regulations apply to the entity. Management is expected to set objectives that incorporate these requirements.

Statement of Condition NEW did not prepare a complete and accurate Schedule of Expenditures of Federal Awards (“SEFA”). The SEFA was substantially prepared by the auditors based upon information obtained during the audit. NEW lacked the internal expertise and procedures necessary to prepare the SEFA. The auditors were required to identify federal awards, determine Assistance Listing Numbers, compile federal expenditures, and prepare the SEFA. Criteria or Specific Requirement 2 CFR § 200.510(b) requires the auditee to prepare a SEFA for the period covered by the auditee's financial statements which accurately presents federal expenditures by federal program and Assistance Listing Number. Questioned Costs Not applicable. Effect Errors or omissions in the SEFA may result in inaccurate reporting of federal expenditures and could affect the determination of major programs and the scope of the Single Audit. Cause Management did not maintain sufficient internal controls over the accumulation, review, and reporting of federal award expenditures necessary to prepare an accurate SEFA. Recommendation Management should establish and document procedures for preparing and reviewing the SEFA, including: • Maintaining a complete listing of all federal awards. • Reconciling SEFA expenditures to the general ledger. • Verifying Assistance Listing Numbers and pass-through information. • Performing an independent management review prior to submission to the auditors. Views of Responsible Officials and Corrective Action Plan Management agrees with the finding and will implement procedures to ensure the SEFA is prepared and reviewed internally prior to the commencement of future audits.

Federal expenditures, including amounts provided to subrecipients, reported to the Illinois Office of Comptroller (IOC) which were used to prepare the schedule of expenditure of federal awards (SEFA), did not agree to IEPA’s financial records provided for audit. Specifically, we noted the following differences between amounts provided for audit by IEPA and the amounts passed through to subrecipients of the CWSRF and DWSRF programs reported to the IOC for the SEFA for the year ended June 30, 2025. "See full report for chart/table." Finally, we noted IEPA’s controls over reporting federal expenditures, including amounts passed-through to subrecipients, were not designed at a sufficient level of precision to ensure complete and accurate reporting in a timely manner. Criteria or Requirement: According to 2 CFR 200.510(b), a recipient of federal awards is required to prepare a schedule of expenditures of Federal awards (SEFA) for the period covered by the entity’s financial statements which must include the total Federal awards expended as determined in accordance with 2 CFR 200.502. Among other things required by 2 CFR 200.510(b), the SEFA must include the total amount provided to subrecipients from each Federal program. Additionally, 2 CFR 200.303 requires non-Federal entities receiving Federal awards to establish and maintain internal control designed to reasonably ensure compliance with Federal laws, regulations, and program compliance requirements. Effective internal controls should include procedures to ensure federal expenditures, including amounts passed-through to subrecipients, are accurately reported on the SEFA and to other State agencies, where applicable. Cause: In discussing these conditions with IEPA management, they stated the differences between IEPA records of federal expenditures and amounts passed through to subrecipients compared to the State’s SEFA were due to inclusion of accrued expenditures and some vouchers in transit at the end of the fiscal year. Possible Asserted Effect: Failure to accurately report federal expenditures impedes the completion of an audit in accordance with the Uniform Guidance which may result in the suspension of federal funding. Repeat Finding: A similar finding was not reported in the prior year audit. (Finding code 2025-006) Statistical Sampling: The sample was not intended to be, and was not, a statistically valid sample. Recommendation: We recommend IEPA establish procedures to accurately report federal expenditures (including subrecipient expenditures) used to prepare the SEFA to the IOC. Views of IEPA Officials: IEPA accepts the recommendation. IEPA will establish additional procedures to ensure federal expenditures (including subrecipient expenditures) are accurately reported.

Federal and State expenditures under the WIC, TANF and CCDF Cluster programs are comprised of programs operated by various State agencies. As the State agency responsible for administering these programs, IDHS has executed interagency agreements with each of the State agencies expending federal and/or State program funds. The interagency agreements require periodic reporting of a summary of the agency’s “allowable” expenditures to IDHS for preparation of the financial reports required for each program. As the State agencies expending program funds do not determine under which program IDHS reports their expenditures, IDHS is responsible for establishing procedures to ensure the expenditures reported by the expending State agencies meet the specific requirements applicable to the federal program. During the year ended June 30, 2025, IDHS reported expenditures from other agencies that were claimed for reimbursement or used to meet MOE requirements as follows: "See full report for chart/table." IDHS’ procedures to monitor other State agencies expending program funds reported by IDHS include the following: Interagency agreements were reviewed and updated (where necessary) to include all State programs claimed under or used to meet MOE requirements of the WIC, TANF and CCDF Cluster programs in its interagency agreement. Program questionnaires were updated where necessary and distributed to each of the State agencies to assist in documenting the nature of the expenditures provided to IDHS and the internal controls established to ensure compliance with the applicable federal regulations. Quarterly certification reports were collected from each of the State agencies to support amounts reported in the federal reports required for each federal program. Expenditure details were obtained from each of the State agencies and were reconciled to the quarterly certifications. However, during our test work over the documentation of the monitoring procedures discussed above, we noted the following deficiencies: Program questionnaires describing internal control procedures for the WIC and CCDF programs were not obtained by IDHS from the Illinois Department of Healthcare and Family Services, Illinois Student Assistance Commission, Illinois Board of Higher Education, and Illinois Community College Board. Rather, for the CCDF program passthroughs a more limited questionnaire was obtained in the State's Grant Accountability and Transparency Act (GATA) Audit Report Review Management System (ARRMS) which is used to complete risk assessments procedures for the State’s subrecipients. Quarterly certification reports were not prepared during the period for the WIC, CCDF, and TANF programs by the Illinois Department of Healthcare and Family Services, Illinois Student Assistance Commission, Illinois Board of Higher Education, Illinois Department of Children and Family Services, and Illinois Community College Board. IDHS did not perform a detailed review of costs claimed from expenditures reported by any of the other State agencies to ensure they met the specific program requirements. The other State agencies do not necessarily know which federal program or maintenance of effort requirement the costs they are providing to IDHS will be claimed or used and are not able to assess whether the costs are allowable. Further, IDHS did not assess whether the expenditures reported by other State agencies were paid during State fiscal year 2025 to ensure the amounts reported to the Illinois Office of Comptroller (IOC) and used to prepare the schedule of expenditures of federal awards (SEFA) were cash basis expenditures. Criteria or Requirement: According to 2 CFR 200.510(b), a recipient of federal awards is required to prepare a schedule of expenditures of Federal awards (SEFA) for the period covered by the entity’s financial statement which must include the total Federal awards expended as determined in accordance with 2 CFR 200.502. Effective internal controls should include procedures to ensure federal expenditures are accurately reported on the SEFA and to other State agencies, where applicable. Cause: In discussing these conditions with IDHS officials, management stated that the current monitoring process was not adequate to ensure interagency expenditures and MOE of federal monies from other State agencies met applicable program regulations as they relate to WIC, TANF, and CCDF. Possible Asserted Effect: Failure to properly monitor interagency expenditures may result in claiming of expenditures that are inconsistent with the objectives of the federal program and federal funds being expended for unallowable purposes. Repeat Finding: A similar finding was reported in the prior year audit as finding number 2024-006. (Finding Code 2025-034, 2024-006, 2023-017) Statistical Sampling: The sample was not intended to be, and was not, a statistically valid sample. Recommendation: We recommend IDHS review its current process for identifying and reporting interagency expenditures and implement monitoring procedures to ensure that federal and state expenditures expended by other State agencies meet the applicable program regulations.Views of IDHS Officials: The Department accepts the recommendation. IDHS will establish additional monitoring procedures, including sampling interagency expenditures and reviews of support documentation to ensure expenditures related to federal grants (including those made by other agencies and those made to subrecipients) are accurately reported in accordance with federal program requirements.

Federal expenditures, including amounts provided to subrecipients, reported to the Illinois Office of Comptroller (IOC) which were used to prepare the schedule of expenditure of federal awards (SEFA) did not agree to IDHS’ financial records provided for audit. Specifically, we noted the following differences between amounts provided for audit by IDHS and the SEFA amounts reported to the IOC for each program for the year ended June 30, 2025: "See full report for chart/table." The following differences were also identified relative to amounts provided to subrecipients for the following major programs: "See full report for chart/table." Additionally, we noted the cash basis expenditures provided by IDHS for our audit procedures included accrued (not paid) expenditures. We also noted these same amounts were reported to the IOC and were used to prepare the SEFA. Specifically, we noted expenditures that were not paid as of June 30, 2025, were erroneously reported as cash basis expenditures for the year ended June 30, 2025: "See full report for chart/table." We also noted the interagency expenditures provided by IDHS for our audit procedures included out of period expenditures. These same amounts were reported to the IOC and were used to prepare the SEFA. Specifically, we noted TANF Emergency Assistance (EA) payments which were claimed in fiscal year 2025 but reported in fiscal year 2024 totaling $14,742,262, and we noted EA payments which were claimed in fiscal year 2026 but were reported in fiscal year 2025 totaling $14,849,458. The net impact is an overstatement of TANF expenditures of $107,196 for the year ended June 30, 2025. Finally, we noted IDHS’ controls over reporting federal expenditures were not designed at a sufficient level of precision to ensure complete and accurate reporting in a timely manner. Effective internal controls should include procedures to ensure federal expenditures are accurately reported on the SEFA and to other State agencies, where applicable. Cause: In discussing these conditions with IDHS officials, they stated the differences in the amounts of federal expenditures and amounts passed through to subrecipients in IDHS’ records compared to the SEFA were due to inclusion of accrued expenditures in IDHS’ records and/or some slight differences related to vouchers in transit at the end of the fiscal year. Also, IDHS’ records for one federal grant program did not initially include non-cash expenditures which had been reported on the SEFA. Possible Asserted Effect: Failure to accurately report federal expenditures impedes the completion of an audit in accordance with the Uniform Guidance which may result in the suspension of federal funding. Repeat Finding: A similar finding was reported in the prior year audit as finding number 2024-014. (Finding Code 2025-042, 2024-014, 2023-012, 2022-010) Statistical Sampling: The sample was not intended to be, and was not, a statistically valid sample. Recommendation: We recommend IDHS establish procedures to accurately report federal expenditures (including subrecipient expenditures) used to prepare the SEFA to the IOC. Views of IDHS Officials: The Department accepts the recommendation. IDHS has established additional procedures to ensure federal expenditures (including subrecipient expenditures) are accurately reported.

Federal Program Information: Student Financial Assistance Cluster (ALN: Various), Child and Adult Care Food Program (ALN: 10.558), and Substance Abuse and Mental Health Services Projects (ALN: 93.342) Criteria or Specific Requirement (Including Statutory, Regulatory or Other Citation): L. Reporting - Per Uniform Guidance, §200.510(b), the auditees receiving federal funding are required to prepare a schedule of expenditures of Federal awards (the “Schedule”) for the period covered by the auditee's financial statements. While not required, the auditee may choose to provide information requested by Federal awarding agencies and pass-through entities to make the Schedule easier to use. At a minimum, the Schedule should list individual Federal programs by Federal agency and provide total Federal awards expended for each Federal program during the entity’s fiscal year. Condition: During our audit procedures, we noted that the College did not prepare the Schedule for the year ended May 31, 2025. Cause: Insufficient internal controls and administrative oversight with respect to the timely preparation and review of the Schedule. Effect or Potential Effect: Lack of appropriate and timely preparation and review of the Schedule by management increases the risk that the amounts, contract or grant names, ALN information, and classification of the awards are presented incorrectly and that federal awards are not managed in accordance with Uniform Guidance. Questioned Costs: None. Context: The College did not prepare a Schedule for the year ended May 31, 2025. Additionally, management was unaware of certain federal awards received during the year. Identification as a Repeat Finding: No similar findings noted in the prior year. Recommendation: We recommend that the College implement additional internal controls and enhance their current policies and procedures to effectively maintain a detailed Schedule that contains the correct amounts, contract or grants names, ALN information and classification of the awards, which can be more readily reconciled to the underlying general ledger detail prior to receipt by the auditors. We also recommend that frequent cross departmental communication is established to ensure that all federal expenditures incurred and federal awards received during the year are appropriately reflected within the Schedule as required. The implementation of these internal controls will ensure that the Schedule is accurate/complete and also ensure that federal awards are managed appropriately in accordance with Uniform Guidance. Views of Responsible Officials: Management acknowledges the deficiency in the preparation and oversight of the Schedule of Expenditures of Federal Awards (SEFA) for the year ended May 31, 2025. To address this finding, the College will: • Implement a formal SEFA preparation policy aligned with 2 CFR §200.510(b) • Develop and maintain a centralized federal awards tracking log identifying: o Federal agency o Program name o Assistance Listing Number (ALN) o Award number o Pass-through entity (if applicable) o Expenditures by fiscal year • Establish quarterly reconciliations between the general ledger and the federal awards tracking log • Require structured cross-departmental communication between the Business Office, Financial Aid Office, Grants Administration, and program departments to ensure all federal awards received and expended are identified timely • Implement documented management review and approval of the SEFA prior to submission to auditors These corrective measures will strengthen internal controls over federal award tracking, improve the accuracy and completeness of the SEFA, and ensure compliance with Uniform Guidance requirements.

SECTION III - FEDERAL AWARD FINDINGS AND QUESTIONED COSTS Finding 2025-001 Federal Award Finding: Material Weakness in Internal Control over Compliance, Material Noncompliance - Reporting ALN and Name: 81.042 - Weatherization Assistance for Low Income Persons Federal Agency: Department of Energy (DOE), Passed through the Alaska Housing Finance Corporation Criteria Per 2 CFR 200.510(b) and Uniform Guidance, the auditee is required to prepare a Schedule of Expenditures of Federal Awards (SEFA) that is complete and accurate, and to provide information reconciling the SEFA to the financial statements and underlying accounting records. Auditors are required to obtain sufficient appropriate audit evidence regarding the completeness and accuracy of the SEFA and underlying population data. Condition During the audit, it was noted that the Organization did not perform reconciliation procedures between the population data used for federal program reporting of direct client expenditures and the financial accounting records. As a result, the audit team performed additional procedures to assess the completeness of the population and identified an unreconciled difference of $8,254 between the population and the trial balance. Cause The Organization did not have adequate procedures in place to ensure that the population data for federal program reporting was reconciled to the financial accounting records. Effect Due to the lack of reconciliation and the resulting unreconciled difference, the auditor was unable to obtain sufficient appropriate audit evidence regarding the completeness and accuracy of the population data for the federal program. This constitutes a scope limitation and may affect the auditor’s ability to express an unmodified opinion on compliance for the program. Questioned Costs $8,254. This amount represents the unreconciled difference between the population and the trial balance. Context The population data consisted of a listing of all clients (homes weatherized by the Organization, including readiness) with expenditures charged to the major program during the fiscal year being audited. The organization’s procedures did not include reconciling the population data used for federal program reporting to the financial accounting records. Such expenditures in the population data consisted of direct client costs which totaled $495,220 and reconciled direct expenditures reported on the Organization's trial balance totaled $503,474, resulting in an unreconciled difference of $8,254 between the population and the trial balance. This deficiency affects the reporting and internal control over compliance requirements for the program. The lack of reconciliation procedures increases the risk that errors or omissions in federal expenditures may not be detected or corrected in a timely manner, potentially impacting the completeness and accuracy of the SEFA. Recommendation We recommend that the Organization implement procedures to ensure that all population data used for federal program reporting is reconciled to the financial accounting records. This should include periodic reconciliations and documentation of any differences identified and resolved. Views of responsible officials and planned corrective actions Interior Weatherization, Inc. agrees with the finding and will implement monthly reconciliation procedures to ensure that direct client expenditures reconcile and adequately support the financial accounting records and reporting submitted to the grantor. See the corrective action plan.

CFR section 200.305 requires Public Housing Authorities (PHA) to minimize the time federal funds are drawn down to expenditure. The Public Housing Capital Fund provides guidelines of three business days from draw down to expenditure to minimize the interest accrued by the PHA. Per review of all Public Housing Capital Fund funding received by the Authority we noted $1,024,894 of received funding identified as not being expensed within three business days of being received. 2 CFR Subpart F § 200.510(b) requires the auditee to prepare a Schedule of Expenditures of Federal Awards (the Schedule) for the period covered by the County’s financial statements which must include the total federal awards expended as determined in accordance with § 200.502. At a minimum, the schedule must: (1) List individual Federal programs by Federal agency. (2) For Federal awards received as a subrecipient, the name of the pass-through entity and identifying number assigned by the pass-through entity must be included. (3) Provide total Federal awards expended for each individual Federal program and the AL number or other identifying number when the AL information is not available. (4) Include the total amount provided to subrecipients from each Federal program. (5) For loan or loan guarantee programs described in § 200.502 Basis for determining Federal awards expended, paragraph (b), identify in the notes to the schedule the balances outstanding at the end of the audit period. (6) Include notes that describe the significant accounting policies used in preparing the schedule and note whether or not the auditee has elected to use the 10 percent de minimis cost rate as covered in § 200.414 Indirect (F&A) costs. Errors and omissions were noted related to various programs on the Authority’s 2025 schedule, including a $816,389 overstatement of expenditures for the Public Housing Capital Fund program. We recommend the Authority implement increased training on the requirements of not drawing down Public Housing Capital Fund funding until either an allowable expenditure has occurred or will occur within three days after the funding was received. We recommend the Authority implement increased training on the requirements for preparing the Schedule of Expenditures of Federal Awards. We also recommend a greater number of quality control reviews to identify errors before requesting funds and preparing the Schedule of Expenditures of Federal Awards.

Finding 2024-002 Schedule of Expenditures of Federal Awards (SEFA) Preparation (Significant Deficiency) Information on the Federal Programs: Research and Development Cluster. Criteria or Specific Requirement (Including Statutory, Regulatory, or Other Citation): Per 2 CFR § 200.510(b), the auditee must prepare a SEFA for the period covered by the auditee’s financial statements. The SEFA must include total Federal awards expended, including Federal awards passed through from other entities, the name of the pass-through entity, and the identifying number assigned by the pass-through entity. Condition: During audit procedures, we identified a Federal pass-through award from under the Research and Development Cluster that was omitted from the preliminary SEFA. The expenditures associated with this award totaled approximately $72,000 for the fiscal year ended December 31, 2024. Cause: The omission resulted from inadequate internal controls over the collection and review of information used to prepare the SEFA. The client relied on a manual process to track Federal awards, and the pass-through award was not included in the final SEFA due to oversight. Effect or Potential Effect: The SEFA provided to auditors was incomplete and did not accurately represent the total Federal expenditures. This could lead to an understatement of total Federal expenditures and result in noncompliance with Uniform Guidance reporting requirements. Questioned Costs: N/A. Context: The SEFA initially provided by FASEB included 45 Federal awards with total expenditures of $1,248,060. The omitted pass-through award represented expenditures of $72,333, or approximately 6% of total Federal expenditures for the year. The omission appears to be isolated to this particular award; however, it was not detected by FASEB’s existing SEFA preparation and review process. Identification as a Repeat Finding, if Applicable: N/A. Recommendation: We recommend FASEB enhance its internal control procedures over SEFA preparation to ensure all Federal awards, including those received through pass-through entities, are properly identified, tracked, and reported. This could include periodic reconciliations between the general ledger and award documentation and a formal review process prior to finalizing the SEFA.
