Finding 2022-001 Special Tests and Provisions Information on the federal program: Grantor: Department of Education Program Name: Federal Direct Student Loans Assistance Listing No.: 84.268 Views of responsible officials and planned corrective actions: Management agrees with the finding described above. The ISMMS Office of Student Financial Services has implemented a combined monthly reconciliation and drawdown process that identifies and resolves discrepancies, as required by the U.S. Department of Education?s Direct Loan reconciliation guidelines under 34 CFR 685.300(b)(5). The process will be detailed in the School?s procedure manual and staff will be trained accordingly. With this new process in place, we will be compliant with the U.S. Department of Education regulations. Corrective Action Plan for Finding 2022-001 We are in receipt of the Findings Required to be Reported by Uniform Guidance, regarding questioned costs and material instance of noncompliance with respect to Activities Allowed/Unallowed and Allowable Costs/Cost Principles. Decatur Hospital Authority?s Chief Financial Officer, Todd Scroggins, is responsible to oversee and implement the corrective action plan. In its Provider Relief period three and period four reporting submissions for the year ended December 31, 2022, the Authority?s reports included the activity of the Authority and their Nursing Home Facilities (Nursing Homes). The reported activity included other PRF expenses, nursing home infection control expenses and lost revenues. There are four separate nursing home management companies that provide services to the Authority?s seven Nursing Homes. There were approximately $358,571 in nursing home infection control expenses that were unable to be reconciled to eligible expenses for one of the Nursing Homes. The Authority provided the Nursing Homes with templates to use to provide the Authority with the necessary information for the reporting as the reporting was complete on the TIN of the Authority. The Authority relied on the accuracy of the information provided by the Nursing Homes. The Authority was not aware of the findings in the audit of period 1 and period 2 at the time the Authority submitted period 3 reporting. Therefore, the inaccurate reconciliation of eligible infection control expenses from period 2 was also used for reporting in period 3, which caused a recurrence in audit findings due to timing of audits and findings reported to the Authority. The Authority?s CFO will judgmentally perform detailed testing of reported costs and lost revenue from the Nursing homes in future reporting periods. In addition, the Authority?s CFO and management team will perform a detailed analysis of the reporting requirements in accordance with the final guidelines set by HRSA for future reporting periods. As deemed necessary, the Authority will modify the policies and procedures over federal grant reporting. The Authority?s CFO will oversee this to ensure that it is accomplished for future unreported periods as of this date. The corrective action plan will be implemented by December 31, 2023.

Again, we will probably not receive these kinds of grants again and something this simple could be a comment and not a finding. I feel that if there are no issues with the actual funding and finances that it could be a comment. FEDERAL AWARD PROGRAMS AUDITS 2022-001 Documentation of Personnel Expenses (Timesheets) Material Weakness Recommendation: Require all employees to complete a contemporaneous timesheet which includes all required Uniform Guidance requirements. Action Taken: On January 1, 2023, an electronic time reporting function was put into effect through ADP (?Automatic Data Processing?), the company?s payroll processing system. This improvement allows employees to enter their time and select a cost center (?department code?) at the time of entry. It then routes the timesheet for approval by the supervisor before reaching the accounting department for payment initiation, resulting in an automated review and approval.

Explanation of disagreement with audit finding: There is no disagreement with the audit finding. Action taken in response to finding: SLRF reporting was a new requirement for the City in 2021. Despite review, preparers missed the expenditure line item on the report. Due to timing, this is still outstanding from the prior fiscal year. The City has since implemented more robust review processes to assure each line item is properly addressed before submittal. Condition: Expenditures totaling $89,150 were incurred during the period March 3, 2021 through March 31, 2022 were not reported in the Project and Expenditure Report submission. Cause: Expenditure invoices were incurred with CSLFR funds were not properly identified by management and subject to the reporting compliance requirement. Recommendation: We recommend that the City implement procedures to identify invoices to be funded by CSLFR funds and report them in the Project and Expenditure Report submission. FINDINGS - FEDERAL AWARD PROGRAMS AUDITS Finding 2022-001 - Special Tests and Provisions - Residual Receipts Excess Federal program information: Title: Section 811 Capital Advance CFDA Number: 14.181 Resolution Status: In Process Information on Universe Population Size: Population size is the total amount in the Residual Receipts account at year-end, June 30, 2022. Sample Size Information: Residual receipts ending balance at June 30, 2022, considering excess residual receipts due for remittance at PRAC contract termination/renewal at June 1, 2022. Identification of Repeat Finding and Finding Reference Number: This is the fourth consecutive year for this finding for the property. Criteria: Pursuant to Housing Notice H-2012-14 and additional authoritative communications from HUD, the organization was required to remit excess residual receipts (all amounts over a prescribed allowance of $250 per revenue-producing units, $3,500) at the time of the PRAC contract termination/renewal, June 1, 2022. Statement of Condition: As of June 30, 2022 the excess residual receipts, $4,861 has not been remitted to HUD. A form 9250 has been submitted to HUD but it is pending as of September 23, 2022. Cause: Management has submitted a request to withdraw the excess funds from residual receipts and submit to HUD, but the request has not been approved and management has not followed up on the original request. Effect or Potential Effect: The project is not in compliance with the Capital Advance and current HUD regulations, the project?s residual receipts account was over-funded for the current year and excess residual receipts have not remitted to HUD as required. Auditor Non-Compliance Code: B Questioned Cost: $4,861 Reporting Views of Responsible Officials: Management agrees that there are excess funds in the residual receipts account. Recommendation: Management should follow up with HUD relative to the approval request to remit excess residual receipts as described. Finding 2022-002 Condition Management's review of the enrollment reporting did not detect errors on certain student data elements. Certain student records within the NSLDS were identified with inaccurate data elements. Corrective Action Plan Corrective Action Planned: In the submission of these students' graduation status, errors in the Alverno dataset resulted in inaccurate transmissions to the Clearinghouse in some cases. Since that time, we have improved our review process. Our current process involves a collaboration between the Registrar and Senior Data Specialist on the Institutional Research team within our Assessment and Outreach Center to ensure that the number of graduating records matches in all reporting processes. This double review provides another opportunity to find and correct enrollment errors before submitting the files to the Clearinghouse. Additionally, the Senior Data Specialist carefully reviews all errors returned by the Clearinghouse and makes corrections to the records as needed to ensure that completions are correctly applied. Finally, campus wide processes to verify enrollment at census and create standardized calendar dates have been implemented to reduce the opportunities for data error. Finding 2022-001 Condition Issue with back-dating withdrawals causing R2T4 calculations to not be calculated. Corrective Action Plan Corrective Action Planned: The College?s Financial Aid department has adjusted how students are identified for an R2T4 calculation. Previously, we would work with a date range weekly. However, if there were status changes made that required changes to dates prior to the weekly reporting range, it would fall outside of our date range. Our new process is to use the first day of the semester as the start of our date range, as this will ensure that we catch all students that need a R2T4 calculation regardless of any academic backdating. Responsible Individuals: Melissa Gale, CEO; Erin Odens, CFO Corrective Action Plan: For the current year a waiver was obtained from the USDA acknowledging that the financial statements were not approved from Board of Directors. Going forward the audit will need to be completed and approved by the Board of Directors prior to submission to the USDA. Anticipated Completion Date: February 1, 2023

Finding 2022-004 Federal Agency Name: Department of Health and Human Services Program Name: COVID-19 Provider Relief Fund and American Rescue Plan Rural Distribution Federal Assistance Listing #93.498 Compliance Requirement: Reporting Finding Summary: The Hospital did not properly include 340B revenues and final audit adjustments in net patient service revenue. In addition, the Hospital did not properly report payor categories for quarters in which the net patient service revenues were negative. Responsible Individuals: Patti Clavette, Chief Financial Officer; Jackie Lundblad, Chief Executive Officer Corrective Action Plan: Management will modify the lost revenue reported on future reports, if any, to reflect an accurate total lost revenue amount. In addition, a formal review and approval process will be implemented to ensure calculations are in accordance with applicable requirements and a member of management will be identified to review all reporting requirements for federal grants and awards to ensure the Hospital is in compliance with the requirements.
