Condition During the year ended April 30, 2026, TEAM incorrectly determined the eligibility of two participants in accordance with the applicable Income Eligibility Guidelines. Cause A review of the Income Eligibility Application for CACFP Child Care Centers form was not performed with sufficient accuracy to ensure participant eligibility was determined in accordance with the applicable Income Eligibility Guidelines. As a result, two participants were assigned to incorrect eligibility categories. Effect or Potential Effect As a result of the eligibility determination not being performed with sufficient accuracy, two participants were assigned to incorrect eligibility categories, resulting in TEAM charging less program fees to the CACFP grant than required under the applicable Income Eligibility Guidelines. Questioned Costs None. Context We selected 40 participants to test compliance over participant eligibility determinations. TEAM did not properly categorize two of the participants tested based on their household income and family size. Identification as a repeat finding This finding is not a repeat finding. Recommendation TEAM should ensure participant eligibility is accurately determined based on household income and family size in accordance with the applicable Income Eligibility Guidelines. A secondary review process should be established to verify eligibility classifications are properly determined and reported to the grantor, and fees are appropriately calculated and charged to the participant.
