Notes to SEFA

San Antonio Regional Hospital receives grants to cover costs of specified programs. Final determination of eligibility of costs will be made by the grantors. Should any costs be found ineligible, San Antonio Regional Hospital will be responsible for reimbursing the grantors for these amounts. Additionally, expenditures incurred for various programs may exceed the amounts awarded from the respective pass-through entity or agency. The amounts reported on the Schedule are limited to the award amounts. Amounts in excess of this amount are paid out of non-federal sources.
San Antonio Regional Hospital incurred expenditures in fiscal years 2020 through 2025, related to four project worksheets; however, FEMA did not approve the project worksheets until fiscal year 2025. As a result, $7,361,928 of Assistance Listing Number 97.036 federal expenditures reported in the Schedule for the year ended December 31, 2025 were incurred in prior years. The SEFA includes $7,376,117 of FEMA Funds obligated by the Department of Homeland Security during fiscal year 2025.

Finding Details

Identification of the federal program: Federal Grantor: U.S. Department of Homeland Security Assistance Listing No.: 97.036 COVID-19 Disaster Grants – Public Assistance (Presidentially Declared Disasters) Award Period of Performance: January 1, 2020 – May 11, 2025 Criteria or Specific Requirement (including statutory, regulatory or other citation): Section 200.303 of the Uniform Guidance states the following regarding internal control: “The non-Federal entity must: (a) Establish and maintain effective internal control over the Federal award that provides reasonable assurance that the non-Federal entity is managing the Federal award in compliance with Federal statutes, regulations, and the terms and conditions of the Federal award. These internal controls should be in compliance with guidance in “Standards for Internal Control in the Federal Government” issued by the Comptroller General of the United States or the “Internal Control Integrated Framework”, issued by the Committee of Sponsoring Organizations of the Treadway Commission (COSO).” Condition: As documented in the prior year finding 2024-001 management indicated they had processes and internal controls in place to review and approve FEMA expenditures submitted to the FEMA Portal. However, management did not consistently retain documentation to evidence the performance of these controls. Management implemented corrective action as of December 31, 2024 to ensure detail approval of individual transactions was retained, however many of the expenditures were from prior years, as allowed for by FEMA, when documentation was not retained, thus the finding is repeating in 2025. Cause: Management represented they reviewed and approved the FEMA expenditures; however, supporting documentation to evidence the review and approval was not maintained. Effect or potential effect: A lack of internal controls over the review and approval of FEMA expenditures could result in unallowable expenses being charged to the federal program. Questioned Costs: None. Context: Due to the timing of the remediation of the prior year finding, we did not test controls in the current year. Total federal expenditures for Assistance Listing No. 97.036 totaled $7,376,117. Identification as a repeat finding, if applicable: 2024-001 Recommendation: The Hospital should refine its process and retain documentation to evidence management’s internal controls in place to review and approve FEMA expenditures submitted to the FEMA Portal. View of Responsible Officials: The Hospital agrees with the finding and has implemented procedures to ensure control documentation is retained.
