Amounts reported in the accompanying Schedules agree with the amounts reported in the related federal and state financial reports.
Of the expenditures of state financial assistance presented in the Schedules, the Foundation provided state awards to a sub-recipient as follows: 24MF0199 Newak School-Based Health Center $690,825
The Foundation contracted with the State of New Jersey Department of Health Division of Family Health Services to provide health and medical services to the uninsured population under the Preventive and Primary Care Program. The Letter of Agreement (“LOA”) pays the Foundation a fixed amount per visit regardless of actual cost incurred. For the year ended June 30, 2025, revenue related to the LOA was $63,384.
