Condition: Documentation of verification of legal name was not present for one program participant in the WIOA Adult Program due to a name change, and documentation of verification of the date of birth to support eligibility could not be located for two program participants in the WIOA Youth Activities program. Cause: The Organization’s employment counselors are required to obtain sufficient documentation before assistance is provided, however procedures were not consistently followed. Effect: There is a risk ineligible individuals could be entered into the WIOA programs, and the cost of the assistance may be disallowed.
